The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community. Today, the CSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. Norfolk CSB seeks team members interested in providing skilled services in a public service environment. Please join us as we instill hope and recovery opportunities for the persons we serve. The NCSB is seeking a Case Manager III in their Forensic Services division. The Forensic Services Division works with individuals involved with the criminal justice system. The Case Manager III will give status reports to the Behavioral Health Docket Team weekly. Maintain the Supreme Court data base weekly or as needed. Face-to-Face contact with client in their home, office, and jail. Links to services and resources in the community. Monitors progress, service delivery, physical and mental stability. Facilitates groups. Department Hiring Salary Range: $46,589-$53,577. Essential functions include but are not limited to: Completes needs assessments to develop individualized treatment plans Provides case management and related services for clients in the Behavioral Health Docket Link consumer to community services and resources Make appropriate referrals for services Monitor service delivery Maintains accurate timely documentation Maintain Supreme Court Data Base Collaborate effectively with outside agencies professionally Manage time and prioritize assignments Facilitate psychoeducational groups Attends court weekly to give status report Performs related work as assigned. Bachelor's degree in a human services field. Two years experience in a human services field related to the areas of assignment. Some assignments may also require specific experience working with various types of client populations. Preferred Experience: Two years' experience working with adults with serious mental illness and co-occurring disorders who are involved with the criminal justice system. Valid Driver's License may be required depending on assignment. Certification in basic first aid and CPR, depending on area of assignment. QMHP-A license preferred. Working Hours: Monday - Friday 8:30am-5:OOPM Signing Bonus: This position is eligible for a one-time $5,000 signing bonus for new hires.
