Retention Bonus after 6 months of continuous employmentCase Worker is responsible for:
  • Participates and works within the multidisciplinary team structure to cultivate and maintain a therapeutic environment. This includes consultation and collaboration with collateral Agency personnel.
  • Responsible for case management including but not limited to admission, service planning, reunification, discharge planning, and documentation.
  • Case load of 8-12 children within the program
  • Flexibility with individual scheduling based on the needs of the clients/children
  • Communicating effectively and consistently with Clinician, Residential Supervisors, School, Aftercare, Medical staff, Guardians, DSS, CSE, OCFS , and any other applicable treatment team members s regarding the treatment progress of an individual youth.
  • Demonstrates applied knowledge of human service/child welfare values, which includes respecting the dignity, individuality and right to privacy of clients and their families/caregivers.
  • Demonstrates competence in dealing with children and families from diverse cultures and ethnicities.
  • Complete all required program paperwork in an accurate and timely manner.
Requirement:
  • Bachelor's degree in the behavioral sciences, human services or social services fields.
  • Child welfare and/or case management experience is strongly encouraged

Position Summary: Case managers are responsible for ensuring that services delivered on a case-by-case, individualized basis are appropriate and are well documented according to OCFS, DSS, and/or CSE requirements, state licensing standards, and CHK's policies and procedures.
