Job Description:
Overview The southern United States carries a heavier burden of new HIV (Human Immunodeficiency Virus) transmissions as compared to other regions of the country. Disparities in access to optout HIV testing, timely linkage to antiretroviral treatment (ART), and HIV prevention resources remain a significant impediment to the universally adopted goals of UNAIDS with 95% of people living with HIV diagnosed, 95% of those diagnosed on lifesaving HIV medications, and 95% of patients on ART achieving viral suppression. CommUnityCare's Sexual Health Program is comprised of a diverse and multidisciplinary team that is dedicated to ending the HIV epidemic by implementing targeted strategies to scale up HIV optout testing, access to HIV treatment services, rapid linkage to HIV primary care, and treatment as prevention while also supporting future retention in care. The Case Manager - HOPWA will provide a wide range of intensive, clientcentered services to persons with HIV/AIDS in a variety of settings to expedite housing stability and engaging patients in early intervention services to ensure engagement and retention in care with the ultimate goal of HIV viral suppression. This Case Management role will be responsible for the provision of grant-funded activities including housing assessment and case management, eligibility for services, and determining short-term rental/mortgage, and utility assistance for eligible households. This position will be funded through a Texas Department of State Health Services (DSHS) grant for Housing Opportunities for People with AIDS (HOPWA) grant and a Health Resources & Services Administration (HRSA) Ryan White HIV/AIDS Program grant. Both grants have a 5-year project period of 09/01/2024 - 08/31/2029 with the possibility of a 5-year renewal. Responsibilities Essential FunctionsDetermines eligibility and re-certification for housing case management, short-term rental/mortgage assistance, and utility aid services.Serves as a subject matter expert on interventions impacting housing access and stability, providing time-limited housing assistance to prevent homelessness and promote stability.Facilitates Ryan White Early Intervention Services (EIS) for newly diagnosed and returning patients. Facilitates warm hand-offs to Case Manager -- HIV Populations for chronic, long-term needs of patients.Participates in weekly care conferencing with supervisor and peer Case Manager for HIV populations to ensure comprehensive management of housing and medical needs for mutual patients.Essential FunctionsRefers ineligible households to appropriate supportive services, assesses their housing status and primary barriers, identifies social service needs, and coordinates warm hand-offs to the Case Manager specializing in HIV populations for patients with chronic, long-term needs.Maintains client files and documents services using electronic health records and case management software. Ensures compliance with federal and state guidelines, supports Service Plan development, and monitors general housing resources for case management clients.Collaborates with eligible households to create budgets and explore strategies for increasing income or reducing expenses. Works with Grants and Finance staff to ensure rental/mortgage and utility assistance payments are processed accurately and promptly, including tasks such as gathering account numbers and obtaining vendor W9 forms.Participates actively in the Sexual Health Program through meetings, client staffing, trainings, workshops, planning activities, special projects, and providing support/training to colleagues.Collaborates with staff, informs clients about available services, and connects them with appropriate resources. Establishes and maintains effective relationships with community resources, including those offering HIV testing, health, mental health, and social services to underserved populations.Stays updated on HIV/AIDS, treatments, risk reduction techniques, chronic diseases, mental health, insurance, benefits, and case management through self-study and training.Adheres to CommUnityCare's mission, Code of Conduct, and IMPACT statement.Performs other duties as assigned by supervisory staff. Qualifications MINIMUM EDUCATION: Bachelor's degree in social work or similar field MINIMUM EXPERIENCE: One year of case managemenet experience required. REQUIRED CERTIFICATIONS/LICENSURE: Valid driver's license. REQUIRED COURSES/COMPLETIONS (e.g., CPR): Basic Life Support (BLS) Obtained through approved American Heart Association
