Responsible for assisting with preliminary patient care in a culturally sensitive manner. This position is designed to provide in-community consumers with assessment and care coordination support. ESSENTIAL FUNCTIONS: Provides in-community support to program linkage and intake scheduling for outpatient behavioral health, nurse navigation and addictions services. Supports appropriate linkages to services by completing preliminary intakes, assessments, including referral to crisis intervention and mobile response services, as needed. Provides navigation services through the continuum of care, assisting with the scheduling of appointments, care coordination. Responsible for maintaining accurate and current consumer records. Communicates relevant demographic and registration information to programs for effective care coordination. Assists clients with understanding the relevant details of visit as requested by either client or treatment team. Facilitates coordination within multi-team and community service as needed to ensure that preventable and health promotion needs are addressed as part of holistic, whole-person, culturally sensitive integrated care. Facilitates care coordination between outpatient and early intervention services. Processes and assists with reporting as assigned by Program Director. Monitors data aggregation to ensure all measures data is being recorded. Participates as a member of multi-disciplinary team towards the development of treatment plans and implementation of services.
