This organization offers a range of services with a culturally sensitive approach including clinical mental health services, social services, and legal advocacy and representation. At Wafa House, our mission is to serve and strengthen individuals who need financial and emotional support and help them feel a sense of community.Purpose: At Wafa House, a case manager will be responsible for completing the four essential levels of case management:intakes/referrals, needs assessment, service planning, and monitoring with situational evaluation. Case managers should be able to maintain a caseload of clients and advocate for them on a regular basis. They would complete a detailed intake form for each client and create a care plan assessing the needs of the client. They will monitor the client and evaluate their situations and develop connections within social services. Duties and ResponsibilitiesCase ManagementComplete detailed client intake form Facilitate patient care by assessing individual needsCreating individualized treatment plans Maintain confidential client records and reports on a timely basis Apricot service tracking (progress notes on each client contact)Network with community agencies, service providers, etc.Provide client advocacy and find resources for clients as neededCreate meaningful relationships with clients for rapport-buildingCoordinating the client's treatment program by making referrals and communicating with relevant collaterals. QualificationsMust relate well to clients in a caring, professional and unbiased mannerMust be fluent in ArabicBachelor's degree in Social Work or related field (MSW is preferred)Effective verbal and written communication Strong and defined sense of professional boundariesEffective analytical and problem-solving skills Ability to exercise sound judgment and decision making Successful in working collaboratively and harmoniously with the Wafa House teamExcellent time management and prioritization skillsAbility to self-govern and work with minimal supervisionProven ability to interact efficiently and communicate effectively with people from diverse backgroundsFlexible work from home options available. Wafa House is one of the only agencies in the tri-state area that provides culturally sensitive services to the Middle Eastern and South Asian communities. Services are available in English, Arabic, and Urdu. Wafa House has grown to be able to serve many different cultures and we currently serve all counties within New Jersey. These services include marriage and family counseling, individual therapy, play therapy, and substance abuse counseling.
