Salary: $52,797.00RADIAS Health is hiring a full-time Case Manager to join the Targeted Case Management team!The Case Manager provides person centered services to promote recovery and community integration of adults with serious persistent mental illnesses and co-occurring substance use disorders within Ramsey County. Our goal is to help them obtain and maintain an optimum quality of life that is consistent with the person's needs and preferences. Case Managers help clients gain access to needed medical, social, educational, vocational and other necessary services as they relate to the client's mental health needs. Hours: Monday - Friday 8 am - 4 pm or 9 am- 5 pm, 40 hours per weekThe Case Manager position is a hybrid model. Some work is done in the community with clients and other work can be done from home. Duties & Responsibilities Develop and implement the treatment plan that is based on the diagnostic and functional assessment which is approved by the clinical supervisor. Assume with other members of Team responsibility for assuring compliance with Minnesota Rule Chapter 9520. Provide or ensure access to needed and eligible medical, vocational, social, educational, financial, legal, and housing and chemical dependency services. Develop an Individual Community Support Plan for each client every 6 months. Schedule face to face visits as dictated by the ICSP. Make appropriate / culturally sensitive referrals and assist clients in gaining access to needed services. Participate in the commitment process as mandated: provide testimony in probate court, write all required court reports, monitor stayed warrants and provisional discharge agreements and assist pre-petition screening process as necessary. File all legally required reports with Vulnerable Adults, the Ombudsman and Child Protection. Assume with other members of team responsibility for providing the services necessary to maintain and enhance each client's ability to live as independently as possible. Develop an individualized emergency support plan coordinated with community emergency support services. Assist client with housing issues and interface with HRA and PHA staff as well as landlords. Attend scheduled quarterly reviews and treatment planning meetings. Collaborate with, support and provide education to family members and others interested in the client's treatment and welfare when warranted. Provide education to clients regarding the symptoms, course and treatment of their illness. Provide team and agency coverage of client needs. Develop appropriate therapeutic relationship with clients. Complete all paperwork, reports, and charting contemporaneously and in an organized manner. Participate actively in on-going professional growth and development, maintain appropriate professional behavior, and participate in appropriate supervision. Transport clients in the community to various appointment Requirements Bachelor's degree in behavioral sciences or related field (if no degree, 30 semester credit hours in behavioral health sciences) and 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders - OR- Masters or other graduate degree in behavioral health sciences or related field -OR- If no degree, must have 4,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders Must have car, auto insurance and driver's license.
