Job Description:

Ohel is seeking Case Managers to work on a field-based innovative team that serves participants transitioning to the community from a higher level of care. The role will involve facilitating transition from one level of care to the community by addressing the preparatory needs of participants in the early stages of recovery, discharge planning, development of daily living skills, and coping mechanisms through group and individual work in the community. The Case Manager provides care navigation and fosters connection and engagement with community-based organizations that promote a sense of purpose, physical wellness, education, employment, socialization, and community involvement.Responsibilities: Collaborate with referring location on transition planning, including a needs assessment of supports essential to stabilizing the participantProvide intensive emotional and practical support to participants as they transition back into their communities and develop comprehensive, person-centered care plansConduct individual sessions with participants emphasizing prevention and preparing for independent community livingWork with participants and their housing providers to resolve clinical issues that are impacting on the participant's ability to obtain, manage, and retain supportive housingProvide extensive and ongoing case management services including information, case assistance, entitlement counseling, referrals, supportive contact, advocacy and crisis interventionKeep detailed and comprehensive records of all contact with clientsQualifications: Bachelor's degree or higher, preferably in a health-related field, or be a New York State Licensed Practical Nurse (LPN)2 years of case management work experience in a social service agency, preferably serving a behavioral health populationKnowledge of mental illness and the needs of individual living with severe and persistent mental illnessKnowledge of techniques for identifying risk, including crisis management techniquesThe ideal candidate is a self-starter who can work independently as well as within a team and has with excellent follow up skills. This full-time position is located in Brooklyn, offers a hybrid schedule, with inpatient, home, and community visits required. Position includes a competitive benefits package. Salary:$55,000-$60,000
