WAGE RANGE: $29.30-$34.10/hourly JOB SUMMARY: Catholic Charities seeks a passionate and dynamic advocate, who is compassionate, trauma-informed, and culturally aware to join us as a Mental Health Case Manager II! The Mental Health Case Manager (I, II) will be embedded in one of Catholic Charities' permanent supportive housing program and will be assigned to manage a case load of clients with mental health needs. The Mental Health Case Manager will report to the Program Manager with oversight from the Director of Behavioral Health. The practitioner will provide services that are rehabilitative and enable the client to develop and enhance psychiatric stability, personal and emotional adjustment, social competencies, community skills and independent living skills. The practitioner will support the client in strengthening skill areas needed according to their treatment plan. Catholic Charities provides services to clients in Hennepin or Ramsey County. ESSENTIAL FUNCTIONS: Conduct individual assessments, assessing functional strengths, resources and needs. Develop individualized, person-centered, trauma informed community support plans. Implement interventions identified in support plan, support client with referrals to external providers, maintain connection with external providers and provide case coordination support. Develop therapeutic relationship with team and client. Supports and advocates on behalf of client in the procurement of services and benefits entitlement, assisting with required paperwork as necessary. Maintains complete case documentation and records, as required by Catholic Charities, the funders, or licensure/regulatory bodies. Ensure confidentiality of all records and interactions. Coordinate services both internally and externally including; county case manager, family/guardian of client, work personnel, medical personnel, etc. Attend supervision, case consultation meetings and participate in team-based decisions regarding all clients and services. Maintain client files, update client files, update treatment plans a minimum of once every six months and do all required documentation as specified in DHS guidelines, utilizing professional writing skills. Responsible for reviewing and ensuring the relevance, accuracy, and timeliness of data and reporting for purposes of accountability and client outcomes. Provide case management to support housing retention, including supporting timely rent payment, renewal of benefits, maintaining a habitable housing unit, etc. May transport clients Complete diagnostic assessments and submit for approval and billing Create individualized person-centered, trauma informed treatment plans Support the co-facilitation of case consultations, utilize expertise to provide support and consultation to housing case managers working with clients experiencing mental health challenges outside of the case managers own case load Other duties as assigned. MINIMUM QUALIFICATIONS: Master's degree social work, counseling, psychology with Mental Health Practitioner or Professional Licensure eligibility by the State of Minnesota (e.g. LGSW, LICSW, LMFT, LPCC, etc.). Completion of a practicum or internship that: (1) requires direct interaction with adults or children served, and (2) is focused on behavioral sciences or related fields. Has a minimum of two years supervised experience in the delivery of services to adults or children with mental illness, substance use disorder or emotional disturbance: or traumatic brain injury or developmental disabilities and completes training on mental illness recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects. Has previously provided licensed mental health care directives and/or treatment plans Has a valid driver's license as they may transport clients. Has the ability to work with diverse and/or low-income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness.
