Description/Job Summary

+ This person will be responsible for conducting the daily activities of Case Management Services for the individuals residing in the Bridge to Independence program. He/she will carry out all assigned service responsibilities to all individuals enrolled in the program.

+ This person will demonstrate a commitment to working with adults with challenging circumstances which may include mental health issues, homelessness and also have the knowledge and ability to provide intensive case management services.

+ This position shall be in compliance with Federal Wage and hour regulations. Scheduled work time is to be flexible so as to guarantee responsive and meaningful treatment and supervision of residents. Hours of work may be on a 'rotating shift schedule' and may include some weekend and holiday hours. Shifts may also require the individual to be on call for emergencies that may arise.

+ This individual will report to the Program Supervisor of the Bridge to Independence Program.

Responsibilities/Duties

+ Provide intensive case management services for all clients.

+ Maintain residential life in a harmonious and close to normal environment as possible, free of tension and stress so that all residents can live together comfortably.

+ Maintain an advocate's role in assisting residents to obtain services that include but not limited to physical, mental, rehabilitation, educational, employment, economic, social, legal and other services appropriate to their individual needs.

+ Assist residents with regard to developing a monthly budget. Encourage residents to begin saving their money and monitor savings account balance once an account has been opened.

+ Assist residents with referrals. This will include scheduling appointments and accompanying a resident to those appointments.

+ Counsel residents to develop a sense of dignity and responsibility and motivate toward independent living.

+ Maintain an assigned case load and present at monthly treatment staffing.

+ Assist the Program Supervisor in evaluating the residents' needs.

+ Provide residents with a sense of acceptance and stability through ongoing contact and supportive services.

+ Develop residential treatment plans which will be approved by the supervisor and Program Manager and document progress in treatment plan notes.

+ Assess individual living skills and work with residents in developing those skills that will assist them in becoming more self-sufficient.

+ Monitor all medications as prescribed by physicians.

+ Attend supervisory meetings as assigned.

+ Participate in ongoing in-service/out-service training programs.

+ Keep accurate records for each resident.

+ All clients will be responsible for rental payments and daily routine maintenance of their room. The Residential Manager will ensure that clients comply with these requirements.

+ Work with all clients to establish good rental payment histories, save funds for transition to permanent housing and develop household management skills. Personal financial management training will help the client identify and secure employment or training, and establish good household financial management practices.

+ Maintain a working relationship and assist in completion of forms and reports for government and private agencies.

+ Provide advocacy, secure short term emergency services, medical assistance, food stamps, and cash assistance as appropriate. Long term needs may include counseling, budget counseling, representative payee services, job search and/or training, employment service, child care, transportation and assistance in securing permanent housing.

+ Supervise transitional housing and supportive services to enable clients to complete the transition to independent living.

+ Develop short term and long term goals together with the client. Such as obtaining paid employment, participating in substance abuse counseling or follow-up, improving basic skills for job readiness, periodic monitoring, and follow-up of progress.

+ Responsible for initial data gathering, periodic reporting, and client follow-up. Focus on gathering indicators, in order to assess program effectiveness in helping clients obtain self-sufficiency. Follow the client's relocation from transitional housing to permanent housing. The indicators are as follows: Obtain and remain in permanent housing, increased skills and or income and achieve greater self-determination.

+ Encourage clients to progress towards self-sufficiency.

+ Any other duties assigned by the Program Supervisor or Program Manager.

Required Qualifications

+ The individual will possess a high school diploma; an associates or bachelor degree is desired preferably in social work or related field, however, experience may be substituted.

+ The individual must be of good moral character, dependable, reliable, and emotionally stable. He/she must be able to pass a physical examination upon employment and annually thereafter. The individual must possess a current/valid Pennsylvania driver's license and must have access to an insured automobile while on duty.

+ The individual must consent to a Pennsylvania State Police clearance and a Pennsylvania Child Abuse History Clearance and FBI Clearance. He/she must provide complete details of any arrest records pertinent to them, regardless of outcome, and must consent to regular drug and alcohol testing.

+ The individual must be trained and certified in CPR, AED and first aid. If the individual does not hold current valid certifications, training will be provided for the individual during the employment orientation period (first 40 hours of employment).

+ The Diocese of Scranton requires that all employees complete an initial VIRTUS Training Program, designed to empower organizations and people to better control risk and improve the lives of all those who interact with the Church. Employees will also be required to monitor the VIRTUS website and complete any internet updates as they become assigned. Employees must also complete all other training programs, out-services, in-services, conferences, seminars, conventions, etc., as assigned by the Program Supervisor.

+ The individual must successfully complete a three-month probationary/evaluation period upon employment
