Job Summary:

The Case Manager is responsible for completing intakes, assessments, and creation of service plans and the overall coordination and delivery of services. The Case Manager will be responsible for the documentation of services provided in our electronic youth file system. The Case Manager is required to maintain a flexible, organized, and efficient work schedule and is subject to work evening hours, weekends, and be on-call.

Essential Functions:

* Conduct initial intake with youth and family/guardian.

* Ensure the timely completion of all Intake paperwork, Assessments, and Individual Service Plans.

* Ensure all daily documentation is complete and entered SWK youth database in a timely and efficient manner.

* Maintain physical and web-based youth files.

* Establish and maintain therapeutic relationships with youths and families.

* Facilitate and/or co-facilitate group skills based and work readiness/educational support groups.

* Provide crisis intervention for youths and their families as needed.

* Provide weekly home visits with youths and families.

* Ensure youths participate in all components or services offered at the program.

* Work in partnership with referring facilities to coordinate the reentry plans prior to the release of the child.

* Develop a comprehensive array of community resources and partnerships such as schools, service providers, natural supports and other community stakeholders which can be of resources for the youth and family.

* Work with the Juvenile Probation/Parole departments and its contracted facility providers to ensure a smooth transition process from the facility to the community.

Other Functions:

* Attend and participate in all required professional trainings and workshops.

* Maintain open and consistent communication with Clinical Program Director or designee, and other team members.

* Transport youths to program activities, court appointments, school hearings or other treatment meetings as needed in personal vehicle.

* Able to react to change productively and manage other essential tasks as assigned.

Qualifications and Requirements:

A combination of one of the following:

* Bachelor's degree in a social service or related field with two (2) years' experience working with at risk youth and their families; may include part time, volunteer, or internship experience.

* A Master's degree in social services or related field.

* Works well independently.

* Computer literate with working knowledge of Microsoft Windows (Microsoft Word, Excel), variety of online database systems.

* Possess good verbal and writing skills.

* Clear background check from appropriate entity and maintain state mandated car insurance.

* Must possess a valid state driver's license, registration

* Well organized, meticulous, and thorough in work habits.

* Up to 60% -75% travel required.

* Bilingual: Spanish & English Preferred.

Physical Demands:

Must be able to obtain First Aid Certification (CPR). Daily use of various office equipment. Bending, stooping, climbing up stairs and lifting to 15 lbs. required to complete daily tasks. Must be able to drive for extended periods of time.

Work Environment:

Ability to work a flexible work schedule. Meeting with youths in their home or in the community as well as in

the institution or placement facility. Required to transport youths to meetings or activities in the community. Work shifts are subject to change. Noise levels vary from mild to moderate. This position may require extended periods of time in an automobile. Exposed to prevalent weather conditions.
