What you will be doing?
The Case Manager is responsible for providing confidential addiction guidance, education and support to individuals, families, and the community, and for promoting healthy lifestyles and healthy choices. Under the supervision of the Director of Clinical Services, and in collaboration with a multi-disciplinary treatment team. the Case Manager ensures post treatment placement to the most appropriate level of care as recommended for each client. The goal of the case manager is to help clients re-enter society and re-engage in their regular lives as soon as possible. They may help clients accomplish this goal by coordinating care with external providers and community resources such as: Family Members, Primary Care Physicians, Psychiatrists, Intensive Outpatient, Partial Hospitalization, Legal Aids, Family Interventionists, University and High School Administrative Staff, and Employee Assistance Program Representatives. They may also help by assisting them with practical needs like finding housing, helping them find employment and arranging for continuing outpatient treatment after being discharged from the residence, if needed.
What tasks are required?
+ Job Type: Full Time; hours determined by Facility
+ Developing the client discharge/transfer plans to ensure movement to the appropriate levels of care.
+ Contacting referral sources, providing case consultation and coordination with referral sources (for example: business development partners, mental health treatment providers, criminal justice agencies, schools, employers, Child Protection Services)
+ Coordinating ongoing care for client housing and therapeutic needs with the BriteLife Recovery team, substance abuse treatment providers, healthcare providers, community resources, and family members.
+ Documenting all case management service notes, assessments, reassessments, referrals and follow-up in the client's clinical record, providing appropriate signatures and dating of such entries, including those made in electronic records.
+ Positively promoting BriteLife Recovery to prospective clients, employees, and outside referral sources.
+ Obtaining previous records that are relevant to the current treatment episode.
+ Collaborating with the client in order to develop a written aftercare plan that is client-centered and recovery oriented, including upcoming appointments and contact information for recommended resources.
+ Providing active case consultation with the multidisciplinary team.
+ Attend individual supervision with supervisor to discuss growth and other client concerns or needs.
+ Under direction of supervisor, establish appropriate therapeutic boundaries, utilization of identified clinical resources to review boundaries, limit setting for behavior, and develop therapeutic relations with clients.
+ Assists in monitoring the therapeutic activities with patients, assist other team members as needed.
+ Work directly with the clients and provide proper recovery support.
+ Special Projects as needed.
+ Positively promotes current organization to prospective clients and employees.
+ Positively promoting BriteLife Recovery to prospective clients and employees.
What we need from you?
+ Bachelors Degree in Behavioral Health Related field preferred.
+ Preferred Certified Recovery Specialist or Certified Family Recovery Specialist certificate.
+ Minimum of two years of experience working in the substance abuse treatment field.
+ Demonstrated computer proficiency.
+ Prior experience working at a healthcare setting preferred.
