The Case Manager role functions in an advocacy capacity by assisting clients experiencing mental health struggles by offering services that are client focused, utilizing a solution focused and trauma informed therapeutic framework. Case managers work collaboratively with a multidisciplinary team of Career Advisors and a Substance Abuse Specialist so that clients receive a comprehensive system of care which removes barriers towards achieving their goals. This position reports to Mental Health Senior Program Manager. What We Offer:$75,000-$80,000/annuallyMedical, Dental & Vision InsuranceRetirement FundProfessional Development TrainingCommuter BenefitsFlexible Healthcare Spending AccountMental Health + Wellbeing Employee Assistance Program What You Will Do: Assess clients' physical and mental wellness, needs, preferences, and abilities and develop individually tailored comprehensive care and treatment plans Coordinate service delivery of resources within CBO community that support stability, mental and physical wellbeing and personal development for clients with needs beyond the scope of Goodwill career services Refer clients to community services and assistance necessary to remove barriers Work as part of an interdisciplinary team with referral partners and employer engagement staff supporting job seekers' success. Maintain proper notes and documentation in clients' electronic record into data systems, and comply with HIPPA and standards of care guidelinesUtilizes client centered therapeutic framework when developing treatment plans, 1:1 meeting with clients.Develop and coordinate workshops and groups that are targeted towards clients' needs. Develop social histories with emphasis upon psycho-social factors affecting client's care and/or ability to effectively engage in service deliveryCreate a step-by-step reentry care and wellness plan for clients that includes, as appropriate, a mental health and/or substance use treatment plan with Goodwill's staff Maintain up-to-date knowledge of programs and services delivered by federal, state and local agencies, their eligibility and application processes Prepare quarterly reports for Goodwill SF Bay funders and stakeholders including outcome data, success stories, and challenges. Represent Goodwill SF Bay's participation in Workforce Development community initiatives including outreach and recruitment.What You Bring to The Role:Bachelor's degree in social work or social sciences OR 2-3 years of clinical case management experience in the social services field working with culturally diverse populations, as well as experience working with clients that have dual diagnosis, mental health diagnosis and substance abuse diagnosis. Preferred LCSW or LMFT, registered with the Board of Behavioral Sciences.Strong advocacy skills to help support clients in achieving their mental health goals. Knowledge of resources and organizations within San Francisco that provide assistance with housing needs, legal issues, mental health support, and substance abuse treatment.Prior experience in conducting psychosocial assessments, creating treatment plans to address mental health goals.Organization skills to manage several different clients at once, understanding of HIPPA compliance, experience with submitting service notes into CRM database.Prior experience in utilizing de-escalation techniques, conducting risk assessments and safety plans.Experienced in facilitating workshops, support groups, providing 1:1 case management service, and referrals to external organizations within the community.Trained in using various models: harm reduction, motivational interviewing and trauma-informed care.Team-orientated and the ability to work well with others in a highly collaborative environment.Understanding of mental health needs of diverse groups; knowledge of cultures, ethnicities, gender, sexual orientations and age groups other than one's own, able to work effectively with others.Knowledge or experience working with clients who are justice impacted.Excellent listening, verbal and written communication skills.Proficiency in MS Office Suite, Salesforce, Google Apps.A growth mindset and a passion for helping people continuously improve.Pursuant to the San Francisco Fair Chance Ordinance, we will consider for employment qualified applicants with arrest and conviction records.
