Job PurposeThe overall purpose of the Case Manager is to provide comprehensive case management services to homeless veterans residing in the transitional housing to which they are assigned whether it be VETS Place Central, Boudicca House, MLK, or Vet's Place North.
Qualifications- A bachelor's degree in social services is required
- Minimum two years of experience in case management, life skills as well as conducting assessment and counseling services
- Valid driver's license and proof of current insurance
- Experience working with Veterans is preferred
- Experience delivering services to SUD, COD, or homeless populations is also preferred
Duties and ResponsibilitiesConduct participant interviews to determine program eligibility and enroll as necessaryDelivery of case management and supportive services and development of an individualized strategic service plan for veteran residentsCounsel residents to identify and then coordinate supportive servicesWork with residents on personal issues both to recognize and take ownership of special needsMaintain resident contact, daily service data, and follow-up documentationEnter detailed progress notes into the HMISCoordinate and facilitate group sessions and classesAttend meetings related to veteran's homelessness, needs, and acquisition strategies and maintain contact with referral sourcesPrepare monthly status and progress reportsMeet as a member of a multi-disciplinary team to examine service issues, make recommendations, etc. . . . Coordinate with team members in the implementation of empowerment servicesInitiate expanded service approaches and expand upon themCoordinate information and referral services to in-house and community resourcesAssist in the facilitation and submission of documents and forms as requiredParticipate in external outreach effortsMaintain resident records and make recommendations for funding complianceOther duties as assigned or requested
