Purpose:The role of case manager involves conducting comprehensive assessments, providing initial support, and guiding clients through the intake process for substance use disorder treatment. Additionally, the role includes developing and implementing individualized service plans, coordinating care, and ensuring clients receive necessary resources and support for recovery and re-entry.Essential Duties and Responsibilities:Assessment and Intake:Conduct initial screenings and assessments to determine the severity of substance use and mental health issues.Gather and document detailed client histories using ASAM diagnostic criteria.Develop preliminary aftercare and treatment plans.Authorize and process insurance for services.Travel to various locations for assessments and intake documentation.Information and Referral:Provide clients with information about available treatment options and services.Refer clients to appropriate internal and external treatment services, including Medication Assisted Treatment (MAT).Assist clients in navigating the healthcare system and securing necessary resources.Client Support and Advocacy:Offer empathetic and non-judgmental support during the intake process.Maintain a caseload of up to 25 clients, meeting weekly and documenting interactions.Advocate for clients' needs within the healthcare system and community resources.Assist clients in overcoming barriers to treatment and provide emotional support.Coordination of Care:Coordinate access to various treatment services, including therapy, medical care, and social services.Collaborate with healthcare providers, therapists, and other professionals for integrated care.Monitor client progress and adjust service plans as needed.Documentation and Record Keeping:Maintain accurate and confidential records of client assessments, intake forms, and service plans.Prepare and submit reports as required by regulatory agencies and organizational policies.Utilize multiple systems for data entry, including ClinicTracker and Microsoft Office.Crisis Intervention:Identify and address potential crises related to substance use or mental health.Provide immediate support and intervention, coordinating with crisis services as necessary.Collaboration and Communication:Work closely with therapists, case managers, recovery support specialists, and other staff.Communicate effectively with clients, families, and community partners.Participate in staff meetings and case discussions.Compliance and Quality Assurance:Adhere to all regulatory and compliance requirements related to substance use disorder treatment.Participate in ongoing training and professional development.Monitor and evaluate intake and case management processes for improvement.Qualifications:Education:Bachelor's degree in Social Work, Psychology, Counseling, Criminal Justice, or a related field. Master's degree preferred.Experience:At least 1 year of experience in a similar role, ideally within substance use disorder, criminal justice, and/or mental health settings.Experience with ASAM, intake, assessments, case management, or client support preferred.Skills:Strong interpersonal and communication skills.Excellent organizational skills and attention to detail.Proficiency in using electronic health record (EHR) systems and other relevant software.Certifications:Ability to obtain Pennsylvania Certification Board (PCB) certification within 1 year of hire.Must be able to obtain FBI, Act 33 & 34 Clearances, Allegheny County Jail Institutional Clearances.Valid PA Driver's License, Insurance, Vehicle.Work Environment:Ability to work in a standard office environment with occasional travel to client locations or other facilities.Ability to work in a corrections-based environment.Schedule: Monday - Friday, with some evenings, weekends, and holidays.
