This position provides only Psychiatric Urgent Care.QualificationsBachelor's degree from an accredited college or university in a related field plus two years of experienceAbility to assess/reassess behavioral health needs and identify early warning signs of behavioral and physical health crises, using MHA's bio-psycho-social assessment.Ability to provide MI to facilitate engagement, and case management services.Ability to coordinate services within EISS with UCC Therapists and outside EISS with other community treatment, and community services providersValid New Jersey driver's licenseResponsibilitiesUses Motivational Interviewing to provide the following services:Provides MI to support outreach and engagement and recovery-oriented services that reflect SAMHSA's 10 Principles of Recovery and Eight Principles of Wellness and Recovery.Provides the full range of case management services to persons in both Tier I and Tier II services, including assessment/reassessment, service planning, linkage, service coordination, and advocacy.Provides outreach/engagement to participants referred by the Screening Center and other emergency providers, families, police, Sussex County Jail, community treatment/ community service providers - onsite at those locations, onsite at EISS's Living Room Drop-in Service, at the individual's home, or another location convenient for individual.Provides outreach, reassessment, and delivery of packaged meds to persons in Tier I who require such support; transportation to the Living Room Drop-in Service for those in Tier I who are need of crisis/intervention/crisis stabilization; at least weekly reassessment for persons in Tier II; and transportation as needed for persons in Tier II to receive evidence-based, skill-building individual and group treatment.Provides reassessment and all other case management services, if the Peer UCM, SUD UCM, or UCM; and collaborates with the Nurse-UCM in providing the Integrated Health Assessment and case management services related to primary care.Provides on-call services to provide in-person assessment/reassessment, linkage to crisis intervention/crisis stabilization, as well as telephonic support; helps individuals in need to access emergency meds, emergency respite, and emergency food.Provides weekend/holiday coverage on rotating basis for 24-hour/7-day access to services.Provides specialized case management services related to substance use and specialized engagement and support as a peer, if the Peer UCM..Completes all required documentation showing consistent and effective provision of MI and other evidence-based services.Attends and participates actively in staff meetingsAttend meetings with program partners/community providers for service coordination.Reports toLead Urgent Care ManagerFully paid medical coverage for eligible staff.Generous 401(k) match.Plentiful Paid Holidays, Vacation, Personal and Sick time.MHAEM is a proud equal opportunity/affirmative action employer committed to attracting, retaining, and maximizing the performance of a diverse and inclusive workforce.
