Posting DetailsPosting Summary Job Title Case Manager Division Student Affairs Office Department VP Student Affairs Office Work Status Full Time Position Category Staff FLSA Exempt Pay RangeSalary Range $70,000 - $80,000 / year Fund Source State Support Job Summary The Case Manager focuses on promoting holistic student well-being through providing support and follow-up to assist students navigating difficult life events and crisis situations, experiencing emerging distress or deterioration in relation to their health, well-being, basic needs, academic progress, and/or other complex situations involving students of concern. The Case Manager facilitates interactions with students in distress and/or navigating crisis situations through trauma-informed care including conducting intake, providing assessment, intervention, advocacy, resources, and referrals for students experiencing challenges. The Case Manager reviews reports of concerning behavior and develops and monitors action plans to facilitate referrals to on and off-campus resources. The Case Manager leads programming, training, and workshops for faculty, staff, and students. The Case Manager facilitates regularly scheduled CARE Team meetings. The Case Manager initiates and develops relationships with community health providers, hospitals, and other community resources.The Case Manager is responsible for consistently reviewing and updating procedures that correlate with national Case Management standards and best practices. The Case Manager is responsible for facilitating and managing the Mental Health First Aid certification and training program on campus for students, faculty, and staff.Job Duties Duties & Responsibilities:Serves as the Case Manager for students experiencing challenges and/or posing a risk of harm to self and/or others as a part of the Student Support and Outreach team in the Dean of Students Office. The case manager serves as a primary point of contact for emergent and walk-in needs and consults and provides support and referrals to campus resources. Utilizes a case management database to review, submit, track, communicate, update, and manage referrals accordingly. Tracks student data and maintains accurate case management records.Establish a comprehensive plan of care and support utilizing appropriate on-campus and off-campus resources for students. Collaborates with campus partners to determine the best course of action for overlapping cases that intersect with various departments on campus. Re-evaluate and adjust the facilitation of care according to updates to the students' case. Facilitates regularly scheduled CARE Team meetings. Tracks students of concern and follows up with students referred for support. Communicate with external healthcare and case management partners when students are hospitalized. Coordinates the efforts of various Provider's and facilitates communication to ensure the student receives quality support.Advocates for students as they navigate the academic, physical health, mental health, and student life landscape by collaborating with on and off-campus providers to ensure needs and resources are understood. Initiates and develops consistent liaison relationships with off-campus resources, health and service providers, social workers, and case managers. Identify and apply for funding opportunities that address homelessness/displacement, food insecurity, legal aid, financial literacy, childcare, and transportation, etc.Create and deliver outreach education and resources. Collaborate with campus and community partners and deliver educational resources and ongoing development opportunities for students, faculty, staff, and families regarding CARE Team, case management, crisis response, and working with individuals in distress. Provide leadership and training related to helping skills, effective referrals, and critical incident response.Assess data with trend analysis for the CARE Team and Case Management and assist with writing monthly and annual reports. Assist with the development and implementation of CARE Team policy and procedures but not limited to outreach and education, campus wellness initiatives, departmental publications, and social media. Assists with student learning /satisfaction surveys and other methods for data collection and assessment of CARE Team services.Remain current and trained on standard and best practices, emerging strategies and interventions, and research at the local, state, federal, and international levels, supporting the reduction of threats to the campus community, connecting students of concern to the support necessary to enable their retention, learning, and success.Assess referral follow-ups including routine referrals made by individual clinicians and integrated team referrals; which also may include post-hospitalization meetings. Performs ongoing research of community services, and continually provides current and accurate information on available resources for students.Responsible for facilitating and managing the Mental Health First Aid certification and training program on campus for students, faculty, and staff. Perform other related duties as assigned.Serves on Divisional and University committees and projects. Serves as part of an on-call rotation. Some evenings and weekends are required. Requested Minimum Qualifications EducationMaster's Degree in Social Work, Counseling, Psychology, or a related fieldExperienceAt least 3 years of experience as a Counselor, Social Worker or Case ManagerSelf-starter who is comfortable working with diverse populations. Must be outgoing and willing to interact with prospective students in a variety of settings.Experience utilizing Case Management SoftwareExceptional interpersonal, planning, reasoning, and organizational skills.Possess diplomacy, tact, and the ability to use good discretion with confidential student information.Excellent verbal and written communication skills to communicate with students, parents, and families, and collaborate effectively with on and off-campus partners.Exceptional customer service skillsAbility to work independently and as part of a teamDemonstrated commitment to diversity and experience with promoting inclusivity requiredAttention to detail and ability to multitaskStrong public speaking skills Other Preferences for Consideration Experience Preferred:Experience working with a college student population and working in a University/college settingExperience working with crisis response processes, responding to crisis situationsExperience building collaborative relationships with internal departments and community organizationsKnowledge of mental health and basic needs services for college students is preferred.Understanding of and sensitivity to meeting the needs of students of diverse academic, socioeconomic, cultural, disability, and ethnic backgrounds.Licensure/ Certification Preferred:Licensed Certified Social Worker- Clinical (LCSW-C) Knowledge, Skills & Abilities ExperienceAt least 3 years of experience as a Counselor, Social Worker, or Case ManagerSelf-starter who is comfortable working with diverse populations.
