We are seeking a case manager who will be responsible for assigning, monitoring, and organizing cases. They will be directly involved in the lives of their clients by coordinating services, and provide resources and assistance. Planning and scheduling services, working with social workers and doctors, and up dating systems is all in a day's work. The case manager must be highly organized, empathetic, compassionate, nonjudgmental, and eager to help.Duties and ResponsibilitiesAccessing client needs, and developing strategies to help the clientProviding support and resources for clients; good working relationship with support networks, government resources, and community resourcesResearching and referring client resources such as health Assigning workers to respond to shiftsEvaluating needs and monitoring services for quality and meeting goalsDetermining the correct course of action when helping clients, whether it be with a social worker, psychologist, or medical doctorCreating programs/service plans to ensure the best outcomesListening resolving issues and providing help advice and measures for health relationships between clients workers and the agencyCaring for the well-being of clients and helping a wide range of clients, including people with serious illnesses, addictions, disabilities or who have been neglectedOrganizing and influencing community and policymakers to develop programs, policies, and services to assist in social workAdvocating and raising awareness on behalf of clients and the needs of the community and local servicesAssisting clients and providing updates within time framesRequirements and QualificationsPossesses superb written and spoken communication skillsExcellent interpersonal skills with colleagues, community leaders, policymakers and othersExcellent time management skills; organized and able to prioritizeMotivated to take on additional community involvement projects and solve problemsComfortable in a fast-paced environment with multiple casesAble to organize and manage large amounts of files, schedules, dates, and informationSelf-directed and able to work without supervisionComfortable with building personal relationships and dedicated to helping othersEmpathetic and supportive with mentorship and leadership skillsComplete requests for informationMust maintain compliance to local, state and regulatory bodiesManage and maintain client profiles on Sandata, WellSky Personal Care and other record systemsLicensed or Registered Nurse or certified trained in managing patient needsProficient computer skills, including Microsoft Office Suite (Word, PowerPoint, and Excel)Able to work nights and weekendsAble to travel to meet with clients in MA, CT and NY
