Job Details Job Location : Commerce Office - Los Angeles, CA Position Type : Full Time Salary Range : $26.25 - $26.25 Hourly Description The Housing Case Manager role is a case management role that works directly with program participants to develop, arrange, and coordinate housing focused case management services through the CalAIM (California Advancing and Innovating Medi-Cal) Community Supports Program. Participants are adults and children with families. The primary focus is to provide comprehensive Housing Navigation and Housing Tenancy case management services to members who have chronic health and/or mental health conditions who may be experiencing homelessness or at risk of homelessness. The Housing Case Manager would be responsible for all case management activities associated with the designated program. DUTIES/RESPONSIBILITIES Develop and implement individualized service plans tailored to each client's needs, including addressing barriers, setting short- and long-term goals, and action steps to achieve housing stability Act as an advocate for clients to ensure maximum benefits and resources are obtained and maintained to ensure clients ability to become self sufficient Provide case management services, including but not limited to housing placements, referrals to resources needed, connection to mental health services, connections to enhanced care management services, substance use referrals, and other services as needed Work with the manager and director to provide constructive feedback on workflows, outreach techniques, and processes. Maintain accurate and timely documentation of all services provided and as required by contracts Assist clients in obtaining vital documentation such as birth certificates, social security cards, and identification Assist clients with locating viable housing options, linkage to interim/emergency housing, and filling out relatable documentation Develop relationships with property managers and landlords as well as local organizations to streamline referrals when needed Attend all internal and external meetings and trainings as directed by leadership Maintain contact with clients utilizing different modalities of contact including but not limited to in person visits, emails, phone calls, and attempt multiple outreach attempts defined by the contract if unable to contact the client Provide ongoing case management services once a client is housed and assist with maintaining housing Educate clients on essential skills like understanding lease agreements, communicating effectively with landlords, budgeting for rent and utilities and other life skills as needed Maintain accurate and timely documentation of all job-related activities. Coordinate transportation and accompany members to critical appointments as needed. Performs other duties as assigned by manager Qualifications REQUIREMENTS : Bachelors degree in social services field preferred. High school diploma required One years experience working with individuals experiencing homelessness, mental health issues, substance use disorders, and physical disabilities Permanent U.S. residency or citizenship Ability to pass criminal clearance background check Ability to successfully pass a TB screening test Valid California state drivers license, current auto insurance, and a good driving record Excellent time management skills, ability to manage multiple tasks, projects and priorities Ability to work a flexible schedule, with evenings and occasional weekends as needed. The typical schedule would be Monday-Friday 8am-4:30pm Bilingual in English and Spanish highly desired but not required To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
