This involves identifying client needs using a person-centered, harm reduction model that supports people who use drugs in their aspirations and recovery journeys. The Harm Reduction Case Manager will participate in mobile outreach, provide prerelease services to incarcerated individuals, and educate and distribute harm reduction supplies – including sterile syringes – to clients. Reports to Population Health Manager. Quality Standards: Professionalism Positivity Productivity Promptness Kindness Qualifications Qualifications: Education: Bachelor's degree in social work, psychology, public health, or related field OR LPN License with 3 years ambulatory care experience Experience: Minimum 3 years case management experience OR 2 years' experience in harm reduction case management One year of relevant experience may be substituted for each year of required education. Demonstrated ability to utilize computers and demonstrated proficiency in Microsoft Office Experience using data management systems Working with incarcerated individuals: 1 year Working in harm reduction: 1 year Desired Skills, Knowledge and abilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Physical Requirements: May require periods of sitting for long periods of time. Will require travel and some out of the area and overnight travel may be expected. Physical ability to get in and out of vehicles regularly. Essential Job Duties: The Harm Reduction Case Manager provides supportive services including case management and patient navigation to Harm Reduction Program clients. This involves identifying client needs using a person-centered, harm reduction model that supports people who use drugs in their aspirations and recovery journeys The Harm Reduction Case Manager will participate in mobile outreach, provide prerelease services to incarcerated individuals, and educate and distribute harm reduction supplies – including sterile syringes – to clients Provide recovery-oriented support services and outreach to Harm Reduction Program participants; Use harm reduction strategies when working with individuals experiencing problematic substance use; Educate clients on safer drug-use practices and other issues that affect their health and wellbeing; Develop and assist in implementing reentry and service plans informed by client goals and values; Connect clients to medical care, dental care, behavioral health care, substance use disorder treatment, housing, and social services through activities including assisting with paperwork, scheduling appointments, coordinating and providing transportation, and accompanying clients to access services; Maintain client files, agency files, statistics, forms, and other record keeping as prescribed Provide recovery-oriented support services and outreach to Harm Reduction Program participants; The above job description is not to be construed as a complete listing of the assignments that may be given to any employee, nor are such assignments restricted to those precisely listed in the description.
