Job Description:
Case Manager As a Case Manager, you will be responsible for managing client cases to ensure they receive all eligible VA benefits and necessary community services. You will monitor client progress, coordinate benefits, and maintain thorough documentation. Your role is crucial in facilitating housing placements, conducting inspections, and supporting clients towards self-sufficiency.Essential Functions:
  • Complete required documentation for clients to access VA benefits.
  • Track application processes and coordinate with VA representatives.
  • Monitor benefits through team meetings, interactions, and direct client contact.
  • Complete and maintain case management forms and HMIS records confidentially.
  • Coordinate client access to community services and complete necessary referrals.
  • Submit and manage monthly TFA (rent/utility assistance) approvals and budgets.
  • Measure household progress using the HUD Self Sufficiency Matrix.
  • Facilitate housing placements, including counseling and landlord relations.
  • Conduct housing unit inspections and provide documentation.
  • Attend all required meetings and training sessions.
  • Maintain personal safety and adhere to safety policies.
  • Perform any other duties assigned by the supervisor.
Minimum Qualifications:
  • Bachelor's Degree in social work, nonprofit management, or related field (Master's degree preferred).
  • Ten years' experience in a nonprofit leadership position (in lieu of a degree).
  • Extensive case management experience, particularly with families experiencing homelessness, substance abuse, and/or mental illness.
  • Working knowledge of the VA benefits system, including eligibility, applications, and appeals.
  • Bilingual skills are preferred.
Other Requirements:
  • Clean driving record.
  • Strong interpersonal skills and effective judgment in interviewing clients and coordinating services.
  • Good writing skills for program planning and reporting.
  • Ability to use independent judgment and perform research and inspections.
  • Ability to remain calm in emergency situations.
