Position Summary:The Case Manager is responsible for completing client intakes for the unit, managing a caseload and provide group facilitation when needed. Case management duties include individual case management sessions, developing treatment plans, documenting client progress for court reporting and making referrals to internal and external services. The Case Manager will report to the unit's Program Director. Essential Duties and Responsibilities:
- Conduct intake for new clients enrolling into the program;
- Perform general case management duties, including weekly individual sessions with clients, referrals to community-based services, developing client schedules, and treatment plans;
- Assist clients in identifying and prioritizing needs;
- Provide referrals to ensure all participants' concrete needs are addressed;
- Ensure accurate and timely data entry of all client information into the agency's database;
- Provide accurate and timely information in court reports;
- Monitor participants' attendance and compliance with mandate requirements;
- Conduct and track client's toxicology;
- Participate in case conferences for optimum collaboration among departments/programs and external agencies;
- Attend and participate in meetings, conferences, and training;
- Conduct group facilitation services when needed;
- Engage in his/her own ongoing professional development;
- Additional responsibilities as required.
QualificationsQualifications:
- One year of case management experience preferred;
- High school diploma or GED/HSE required;
- Experience and commitment to working with mandated participants with criminal justice involvement;
- Strong Time Management Skills;
- Strong Microsoft Office and database entry skills;
- Strong assessment and engagement skills;
- Strong written and verbal communication skills;
- Facilitating group sessions;
- Bilingual English/Spanish a plus;
- Strong organizational and leadership skills;
