Overview Under the direction and supervision of the Program Director (or designee), the Clinical Case Manager provides targeted discharge linkages for clients to ensure ongoing stabilization for the next appropriate level of care. All aspects of service delivery are to be conducted in alignment with the Crisis Now Model No Wrong Door/No Force First approach. Services are tailored for youth (ages 7-17) and their families**and are not to exceed 23 hours. 23 Hour CRC provides rapid assessment/s, crisis stabilization and discharge coordination for the next appropriate level of care. Case Manager is responsible for maintaining up-to-date resource lists and developing partnerships with external parties to ensure effective linkage with a warm hand-off to support crises stabilization. All work is performed within the Crisis Now Model** and systems-based framework with strong collaboration skills with and in support of other services offered by St. Joseph's Villa and the community (partnership with Region 4), in accordance with the highest professional ethical standards, and in alignment with Saint Josephs Villa mission, values, and guiding principles. **Crisis Now Model - (all persons requesting services regardless of age, etc. are to briefly be screened and linked through a warm hand-off to appropriate level of care). Responsibilities Coordinates community linkages/resources for all walk-ins, requiring either screening and linkage or admissions to the 23-Hour CRC. Coordination of all assessments once a client is admitted into the program (i.e., medical, mental, substance, etc.) Provides high-level collaboration for clients and families to support stabilization, warm hand-off to next level of care in coordination with the crisis team. Works directly with and in conjunction with crisis team to identify client needs and most effective treatment for continuous stabilization upon discharge. Works with 23-Hour treatment team to develop crisis safety plans, and discharge planning based on the client and familys prioritized needs. Researches community-based organizations and governmental agencies to determine what resources are available to support the clients needs. Actively links clients and their families to these identified resources throughout service provision. Maintains case management records. Compiles information to prepare discharge summary reports on clients. Maintains contact with designated family members or guardians and school systems. Coordinates community support and intra-agency support as needed for each individual client. Maintains smooth processes and coordination for the tele-medicine clinic to include scheduling, maintaining equipment, preparing documents for medical staff, etc. Administers medication, oversees compliance with medication logs, and storage of medications. Prepares regular narrative, tabular, and statistical reports for St. Josephs Villa management staff, Service Board, and licensing, accrediting, and regulatory agencies. Prepares data for accurate billing for services, complying with the requirements of the varied funding streams. Uses approved behavior management techniques to work with clients in crisis. Models high level of professionalism with professionals to include behavior and dress. Participates in staff meetings and other staff and program group activities, including after school and evening programs, essential for the operation of an effective psycho-educational program. Evaluates work processes and methods and submits recommendations for continuous quality improvement. Attends scheduled training programs for professional development that includes, at a minimum, trainings required by St. Josephs Villa and by regulatory and accrediting bodies. Participates in the on-call rotation. Qualifications BS/BA degree in Psychology, Human Services, or related field. Two years related experience. One year of clinical experience with children (QMHP-C preferred). Proficient with Windows, Outlook, MS Office including Word, Excel and PowerPoint. Experience with proprietary software preferred. Valid Virginia drivers license with excellent driving record.
