Description
Join Callen-Lorde Community Health Center, a leading LGBTQ-focused health center, as a Case Manager. We are seeking a compassionate and dedicated professional to support our diverse patient population in accessing comprehensive healthcare services.
As a Case Manager, your responsibilities will include:
+ Conduct comprehensive assessments of patients' needs, creating individualized care plans.
+ Collaborate with healthcare providers and community resources to ensure a holistic approach to patient care.
+ Advocate for patients' rights and coordinate access to LGBTQ-affirming services.
+ Facilitate support groups and educational sessions on relevant health topics.
+ Maintain accurate and confidential case records, ensuring compliance with HIPAA regulations.
+ Provide crisis intervention and emotional support to patients facing challenging situations.
+ Collaborate with multidisciplinary teams to enhance patient outcomes.
Work Schedule/Salary:
This is a full-time, 5-day-per-week position including some evening hours. You must be available for early morning, evening, and/or weekend shifts.

Qualifications
+ High school diploma or GED preferred.
+ In the absence of education credentials, experience in social services, case management, and/or health care will be considered.
+ Familiarity with LGBTQIA, HIV and health-related issues and community resources.
+ Strong computer competency, including knowledge of Electronic Medical Records, Computerized Appointment/Billing Programs and other Microsoft Office Programs.
+ Strong customer service orientation.

+ Ability to work independently yet be team focused.
+ Ability to remain calm, focused, and helpful under stress.
+ Knowledge of Microsoft office programs and utilization of these tools for scheduling and communication.

+ Bilingual (Spanish/English) preferred.
