Summary: The Behavioral Health Case Manager provides support to clients, clinicians and psychiatric staff, facilitates the authorization process, scheduling, and helps to coordinate care among Southwest Community Health Center sites and in the community. SPECIFIC DUTIES AND RESPONSIBILITIES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA): Document communications in electronic health record Schedule all psychiatric evaluations and follow up appointments Collaborate with treatment team to provide referrals as needed (i.e., internal medicine, etc). Manage all case management related tasks including responding to in-basket tasks/communications and patient requests in electronic health record Conduct and follow protocols for prior authorizations, nursing orders, medication authorizations. Monitor med management patients for treatment plan updates and discharges. Review client charts for consistency and treatment compliance and communicate regularly with psychiatric provider and clinicians. Conduct vitals (height, weight, blood pressure, pulse) per HEDIS measures. Assist patients with access to community supports/resources. Other duties as assigned. Qualifications: Experience working in a behavioral health and/or medical environment. Knowledge of major psychiatric illnesses. Excellent communication, writing and verbal skills. Familiar with Electronic Medical Record documentation. CPR/BLS Certified Strong interpersonal skills and ability to positively contribute to the greater team Highly desirable: Fluency in a second language Education and/or Experience: Bachelors Degree required Physical Requirements and Work Environment: The physical requirements and work environment described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Generally, the working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials. This person must have the ability to travel as required to work with staff.
