We are a Community Mental Health Center seeking a Case Manager to support clients in navigating the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM-CANS), Social Security Disability Insurance (SSDI), and other social service programs. In this role, the Case Manager will assess client needs, develop tailored action plans, and coordinate resources to enhance their well-being and access to essential services. This position is vital in guiding clients through complex processes, ensuring thorough documentation, and advocating to secure the necessary services and support.
Key Responsibilities:
Client Assessment & Planning:
- Conduct comprehensive assessments to understand clients' needs, health status, financial situation, and eligibility for various services.
- Develop and implement individualized action plans to address clients' needs, including access to social, financial, health, and legal resources.
Case Management & Coordination:
- Guide clients through the IM-CANS & SSDI application process, ensuring that all required information and documentation, such as medical records and work history, are accurately collected and submitted.
- Act as a liaison between clients and external agencies, advocating on their behalf for resources like Medicaid, Medicare, or other assistance programs.
- Maintain consistent communication with clients, providing updates on their application status and other service-related inquiries.
- Monitor and manage discharge plans, including coordinating with healthcare providers, third-party payors, and other stakeholders to ensure a smooth transition and continuity of care.
Documentation & Compliance:
- Maintain detailed, confidential, and accurate case files, documenting client interactions, action plans, and follow-up actions in compliance with company policies and legal regulations.
- Review and update clients' plans regularly to reflect changes in circumstances or needs.
Collaboration & Team Support:
- Collaborate with other case managers and healthcare team members, including physicians, nurses, and administrative staff, to ensure comprehensive, high-quality care for clients.
- Support and guide other team members, reviewing applications and documentation when necessary and helping address any questions or challenges.
- Participate in team meetings, offering strategies and feedback for improving the case management department's processes and client outcomes.
Qualifications:
Education:
- Bachelor's degree in social work, healthcare, human services, or a related field preferred.
Experience:
- Minimum of 2 years of experience in case management, healthcare, social services, or a related field. Experience with IM-CANS & SSDI applications and familiarity with Medicaid/Medicare processes are highly desirable.
Skills:
- Strong organizational skills and attention to detail.
- Excellent communication and interpersonal skills, with the ability to build rapport and advocate for clients effectively.
- Ability to handle sensitive and confidential information with professionalism and discretion.
- Proficiency in Microsoft Office Suite and general office software; experience with case management systems is a plus.
- Ability to work independently and manage multiple tasks simultaneously in a dynamic environment.
- Bilingualism in Spanish preferred

Hours:
- Full-time position, Monday to Friday, 40 hours per week, with occasional flexibility for client meetings outside regular office hours.
