How you can make a bigger difference: PURPOSE: The Jewish Boards Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally affirming, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities. The ACT team provides an integrated set of evidence-based treatment, rehabilitation, case management, and support services delivered by a mobile, multi-disciplinary mental health treatment team. ACT supports clients recovery through a highly individualized approach that provides clients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects. ACT services are developed through the person-centered service planning process and adjusted through the process of daily team meetings. POSITION OVERVIEW: This role is integral to the Assertive Community Treatment (ACT) Team, dedicated to providing comprehensive support for individuals grappling with severe and persistent mental illness (SPMI) and substance use disorders (SUD) living in NYC Mental Health Shelters and/or living in supportive housing programs or their own residences in the community. As an ACT Team Case Manager, you will work proactively in the field, engaging with clients to assess their risk and needs, develop individualized care plans, provide care management services, track and arrange appointments, educate clients and coordinate other aspects of clients health and community services. KEY ESSENTIAL FUNCTIONS: Provide all face to face, in person visits in the community, such as clients homes, shelters, residential programs, hospitals or other locations as client prefers Conduct comprehensive psychosocial assessments, assess for risk for substance use, suicidal ideations and aggression, and treatment plans, laying the groundwork for targeted interventions. Collaborate with the ACT Team, ensuring a minimum of 6 monthly visits to each client for continuous connection and responsive care. Integrate medical, specialized, and behavioral health services into robust and person-centered care plans. Periodically reassess clients' medical and behavioral health needs, treatment adherence, and the imperative for care coordination. Attend daily team meetings, weekly supervisions, and mandatory trainings, actively contributing to collaborative efforts and staying at the forefront of best practices. Lead or actively contribute to team in-service training sessions, driving the continuous professional development of the entire team based on identified needs. Proactively collaborate with clients, their families, and community partners, jointly establishing and pursuing treatment and recovery goals. Coordinate with health and community services, ensuring a unified and integrated approach to client care as well as working with clients towards achieving goals and step down to alternative services. Demonstrate exceptional communication skills, facilitating effective collaboration within the multi-disciplinary ACT Team. Exhibit strong engagement skills, ensuring meaningful connections with clients in diverse settings, including their homes and communities. Adapt and respond effectively to the evolving needs of clients, promoting a holistic and dynamic approach to mental health care. Use evidence-based treatments in the areas of mental health and substance abuse to address client needs-(e.g., cognitive-behavioral therapy, harm reduction, motivational interviewing, trauma-informed-care. Provide services to assist clients with needs such as family support, housing, entitlements, employment, education, vocational interests, community integration, and connection to physical healthcare. This includes escorting clients to various appointments such as medical appointments, HRA, court, job interviews, grocery stores and other as needed. Provide services to assist clients with needs such as legal advocacy, navigating the criminal justice system, and coordination with legal agencies. Following OMH regulations in regards to incident reporting, AOT monitoring and CAIRS Provide after-hours emergency coverage via cell phone on a rotating basis, demonstrating a commitment to continuous support and crisis intervention CORE COMPETENCIES for the position include: Team player who functions well in a multi-disciplinary team environment Reliability and discretion Adaptability Communication, negotiation and relationship-building skills. EDUCATIONAL/TRAINING REQUIRED: At least 18 years of age; High-school diploma or equivalent and have at least six months of direct care experience with individuals with serious mental illness (SMI). Bachelors Degree, preferred EXPERIENCE REQUIRED/LANGUAGE PREFERENCE: Experience working with clients with severe mental illness or substance use; Ability to work independently; Understanding of behavioral health and medical systems and experience with coordination of services; Fluency in Spanish a plus. COMPUTER SKILLS REQUIRED: Prior experience with electronic health records and Microsoft Office VISUAL AND MANUAL DEXTERITY: The candidate should be able to read paper and electronic documents and perform significant data entry into various computer programs. Travel independently using public transportation. Able to input data into the Electronic Health Record. Limited applications of manual dexterity and hand-eye coordination. WORK ENVIRONMENT/PHYSICAL EFFORT Must be comfortable with public transportation, traveling in the community, as this position requires 80% of the time in the field and 20% in the office. This individual must be comfortable walking and standing for long periods of time. Physical Demands: Regularly required to speak clearly and hear the spoken word well. Regularly required to physically operate routine office equipment such as telephones, computers, Regularly required to utilize near vision ability and to read data and documents, including spreadsheets, reports and Electronic Health Records in printed form and on computer screens. Regularly required to travel to various locations for field work in the community. Work Environment: Noise level is consistent with levels usually present in an office, education, rehabilitation or health related environment when in office. To perform the essential functions of this job the candidate must be able to travel within New York City carrying equipment such as a notebook, forms, laptop, mobile hotspot and cell phone weighing up to approximately 10 pounds. To perform the essential functions of this job, the candidate is routinely required to sit (60% of the time) and stand (20% of the time), and travel to and from appointments using public and private transportation options (20% of the time). Risks/hazards associated with the position are those which may be encountered traveling around New York City.
