Job Summary We are searching for a self-disciplined, well-coordinated, and diplomatic professional. to join the CK Family. As a Case Manager at CK Family Services, you will develop and implements program structure and design for the foster care and foster care adoption program. You will supervise foster/adoptive families to ensure compliance with state licensing and monitoring standards, Foster Care and Adoption Contract Terms, Covenant Kids mission, policies, and procedures. You will partner with families to lead and assist the prospective foster to adoptive parents and their potential adoptive child through the adoption process. G e n e r a l R e s p o n s i b ili t i e s Children Complete accurately and update timely all paperwork required to manage childs care in the home. Regularly review all documentation provided by foster/adoptive parents and outside agencies and all other documentation relevant to assigned children and families upon receipt. Make immediate written request for missing or necessary documentation. Follow up until obtained. Immediately notify supervisor of issues. Regularly oversee childs progress and make regular reports to supervisor. Effectively coordinate provision of all therapeutic support services for children. Collect all documentation relevant to those services. Interview all children privately regarding satisfaction with placement, foster/adoptive family and home situation. Immediately notify supervisor of issues. Attend all PCs, court hearings, and family group conferences as required. Attend all ARDs as required. Attend all psychiatric appointments as required. Act as liaison with school personnel, children and foster/adoptive parents. Report suspected child abuse or neglect, immediately to the Abuse and Neglect Hotline and Case Management Supervisor. Effectively communicate with DFPS regarding the child. Thoroughly and timely complete investigations as required. Maintain regular and effective communication with supervisor about all incidents and report such immediately. Foster/Adoptive Family Effectively manage foster/adoptive home and monitor family for compliance with all required regulations making regular reports to supervisor. Maintain regular and effective communication with the foster/adoptive family, keep them informed and hold them accountable for childs service plan requirements. Make regular visits to the foster/adoptive home to observe family interactions and the home environment. Immediately notify supervisor Consistently work to develop the foster/adoptive parents in the areas of teaching and encourage the use of therapeutic behavior interventions. Routinely teach and train foster/adoptive parents the appropriate use and completion of agency forms. Regularly monitor foster/adoptive parent verification relative to the capacity of the family home. Provide regular verbal and written feedback to the foster/adoptive family. Provide verbal and written counsel to the foster/adoptive family as needed. Act as liaison between childs DFPS and foster/adoptive family. Adoption Follow CK Adoption Processes including, but not limited to: Assist in coordination of and attendance of adoption staffing, adoption presentations and placements, and finalization of the adoption. Accurately and timely preparing Adoption Home Study Updates and Post Placement Adoptive Reports as required; reporting findings to supervisor in a timely manner. Professionally presenting and introducing prospective adoptive families to Child Protective Services in Selection Staffing as requested. Preparing and completing all required adoption paperwork accurately and timely. Effectively assisting adoptive parents in researching and applying for adoption resources, such as subsidies, referrals for post-adoptive services and therapy. Effectively conducting face-to-face visits with adoptive family after placement date, including 14 day visit and monthly visits with all household members and document each visit within required timeframes. These visits are supervision of adoptive placement visits. Providing monthly documentation according to agency processes of supervision in the post placement process until consummation occurs. Ensuring service plan addresses adoption at the time of the adoptive placement, and if not already in existence or if expiring, creating and implementing updated service plan within 30 days of placement. Serving as liaison with adoptive familys attorney and DFPS to prepare for finalization. Complete applicable court paperwork accurately and timely necessary for adoption consummation. General Serve in the on-call rotation for after hours and weekend coverage as well as backup daily coverage as needed. Provide crisis management to families as assigned implementing clinical strategies as learned through training attended. Attend trainings as scheduled. Participate in meetings and chart audits as scheduled. Complete and report documentation timely and accurately. Open and process mail / e-mail daily in a timely manner. Answer phone, collect messages and respond to requests timely and accurately. Input data into computer accurately and timely. Represent Covenant Kids in the community by attending meetings and gatherings as required. Assist in staff and foster/adoption parent training as required. Attend all clinical meetings and department meetings as scheduled. Advocate for foster/adoptive parents and children with other agencies as well as Covenant Kids staff. Maintain compliance with all Covenant Kids policies, procedures, and regulatory requirements including all state and federal laws. Review and supervise maintenance of complete and current records on each child and family in compliance with Child Placing Minimum Standards. Maintain confidentiality as required by government law and regulations. Perform other duties and tasks as requested. Qualifications Q u a li f i c a t i o n s Bachelors degree Work deals mostly with areas such as preparing and reading data and figures and visual inspection involving small details. Visual work is primarily close to the observer. Depth perception and field of vision (peripheral) are important but not as critical as the ability to distinguish small details and markings very near to the observer. Excellent organizational and follow-up skills. Excellent interpersonal skills and demonstrated ability to effectively communicate orally and in writing with a variety of people; maintain confidentiality Ability to be on-call as necessary and respond immediately to on call pager or phone. Must have reliable transportation. Ability to handle multiple tasks; self-starter; energetic; detail oriented. Good character, personal attributes and mannerisms to be a good adult role model for residents and their families, and as a professional for case managers under supervision. Ability work effectively under stress and remain flexible in changing situations. Proficient in basic computer skills that include but are not limited to data entry, MS Office, jobsite portals and company software. Ability to relate to multiples ideas and the concepts/theories behind the ideas; ability to remember multiple verbal and written tasks/assignments over several days and occasionally up to several months. Perform the physical demands of the position Ability to use up to 100 pounds of force occasionally, up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people; Ability to walk frequently; stand or sit for prolonged periods of time; squat occasionally, climb stairs infrequently, and kneel and twist as needed. Sufficient good health to properly fulfill all role responsibilities. Employees shall not be permitted to work who have infectious diseases or skin lesions for the duration of the communicability. Ability to perform moderately difficult manipulation such as writing, typing, foot and hand-eye coordination required.
