Job Details Job Location : 60 Lafayette Street, New York, NY, 10013 - New York, NY Position Type : Full Time Education Level : Bachelors Degree Salary Range : $34.00 - $34.00 Hourly Travel Percentage : Up to 25% Job Category : Human Service Clinical Case Manager EAC Network, a not-for-profit social service agency that empowers, assists, and cares for 62,888 people in need through 100 programs across Long Island and NYC, seeks a Clinical Case Manager to work full time for the Manhattan Diversion Court Case Management & Recovery Support Services program. The Clinical Case Manager position is a non-exempt position paying $34 per hour, Monday and Tuesday 9AM- 5PM. The office is located in Manhattan, NY. The role reports to the Program Supervisor. Primary Purpose of Job: The Clinical Case Manager is responsible for assessment, referral and case management of co-occurring participants involved in Manhattan Diversion Court Case Management & Recovery Support Services program.. Principal Duties & Responsibilities: 1. Conduct structured clinical assessments and refer participants to appropriate community treatment providers.. 2. Complete treatment plans and present to the Courtin case conferences 3. Arrange for and implement placement into appropriate treatment programing in a professional manner. 4. Assist clients in obtaining benefits as indicated, including public assistance, SSD/SSI, housing, medical and other needs. 5. Maintain complete client records and have available for inspection by Supervision at any time. 6. Conduct Government Performance Results Act interviews (GPRA's) 7. Conduct evidence-based interventions including but not limited to cognitive behavioral interactive journaling. 8. Maintain caseload through ongoing client contact. 9. Work collaboratively with the treatment court team. 10. Make appropriate site visits to treatment agencies as required. 11 Participate in on-going training both virtual and in-person as delegated by Supervision. Knowledge, Skills & Abilities Required: Bachelor's degree in criminal justice, social work, counseling, forensic psychology, psychology, sociology, or criminology or CASAC with significant experience in Substance Abuse, Mental Health or Criminal Justice. Must have knowledge of treatment programs. Good communication skills, both oral and written. Ability to work cooperatively with treatment and mental health systems. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
