Title: Vital Case Manager Onsite at REACH Belltown location Salary range is: $72,500 - $74,500 ETS strives to achieve pay equity and transparency as part of our goal to create an equitable workplace. The REACH mission is to foster community health and safety through outreach, relationship, healing interventions and systems advocacy for people who use drugs. REACH provides outreach-based-care coordination, multidisciplinary clinical services, and supports to access and maintain housing. All services are based in principles of harm reduction that offer respect and dignity to individuals moving through stages of change in their lives. REACH incorporates a racial equity lens that includes naming the impact and actively dismantling systems of oppression rooted in White Supremacy, while addressing the root causes perpetuating historical trauma and immense suffering in individuals lives. Overview of Vital Program The Vital ( Familiar Faces ) Program is an interdisciplinary care management team providing integrated services to adults experiencing behavioral health challenges (mental health and/or co-occurring substance use), have experienced chronic homelessness, and have frequent contact with the criminal justice system. Vital is comprised of providers from Harborview and REACH/ Evergreen Treatment Services who deliver low-barrier medical, mental health, and case management support services in the community. We strive to offer trauma informed care and a harm reduction approach in addition to evidence based practice. Job Summary: The Vital Program of REACH serves people in King County who are frequently arrested and aims to transform the criminal legal system through community partnerships, advocacy and the empowerment of those most directly impacted. The Vital Case Manager will maintain a caseload of 15 clients and work in collaboration with a multi-disciplinary clinical care team including a primary care ARNP, a psychiatric ARNP, a registered nurse, an occupational therapist and a behavioral health counselor. With the permission of the program participants, case managers provide legal advocacy in the courtroom and coordination with attorneys to mitigate the harms of the criminal legal system. This unique team integrates criminal legal advocacy with harm reduction healthcare to improve the quality of life of program participants. To learn more about this county initiative visit the website: Job Functions Include: Providing re-entry support, engagement and intensive case management to adults experiencing behavioral health challenges who have been repeatedly incarcerated and not effectively engaged in needed services Developing rapport and a trusting relationship with clients through respectful outreach and client-centered engagement practices Providing advocacy and support for participants within the criminal legal system including court appearances and written communication Providing street-based outreach, meeting clients where they are, either on the streets or at other service facilities (including jail, hospitals, crisis facilities and court), to offer services and establish a working relationship Assisting clients in obtaining housing and maintaining housing stability Assisting clients with establishing and maintaining primary medical care and addressing chronic health conditions Assist clients in obtaining State and Federal benefits Working in a fast-paced environment with people whose behavior may be affected by substances, untreated mental and/or physical health challenges. Advocating for the client with a wide variety of other service providers Identifying gaps and barriers in available community resources and advocate for systemic changes Developing and maintaining collaborative relationships with all Vital partners including King County and City of Seattle Prosecutors Office, Harborview and Plymouth Housing Group. Attend all Vital Program staff meetings and other required meetings Requirements Requirements High school diploma or equivalent Three years in social work or outreach programs Demonstrated ability to provide street-based outreach and engagement services to individuals experiencing homelessness, mental illness, trauma and marginalization Local travel on behalf of the agency; including comfort providing frequent in-reach to jails and hospitals Understanding of substance use disorders and harm reduction strategies along with a demonstrated passion for serving people experiencing homelessness and co-occurring behavioral health conditions Commitment to racial equity and social justice Check Out ETS Excellent Benefits - Part of Your Total Rewards Package in this Role!
