Description*$500 Sign-on Bonus!*Options for Community Living, Inc. is looking for a Case Manager to join our Mental Health Licensed Program! An ideal candidate should meet the following requirements:
  • High school diploma and two years of qualifying experience*, or a Bachelor's degree and one year of qualifying experience*.
  • Excellent time management skills required.
  • Valid driver's license, safe driving record, access to a vehicle and valid auto insurance is required.
*Qualifying experience: Verifiable full or part time case management or case work with persons with a history of mental illness, homelessness, or chemical dependence.Our Company Benefits include:

Salary Range:$40,040.00/year ($22.00/hour) - $50,960/year ($28.00/hour)Salary offers will be commensurate with experience and other qualifications.Schedule (35 hours/week):
  • Monday/Tuesday/Wednesday, 9:00 AM - 7:00 PM and Thursday, 9:00 AM - 6:00 PM OR
  • Tuesday/Wednesday/Thursday, 9:00 AM - 7:00 PM and Friday, 9:00 AM - 6:00 PM OR
Location: Apartment Treatment Program sites throughout Suffolk County ORFloat; All Community Residences and an Apartment Treatment Program site throughout Suffolk CountyPay Type: Non-exemptResponsibilities:The Licensed Program Case Manager provides supportive case management services to individuals with mental illness in the apartment treatment program. Monitors maintenance of sites and provides on-site visits to scattered housing locations.
  • Plan and evaluate service plans and monitor objectives. Write progress notes on service plan weekly.
  • Document services provided with appropriate notes.
  • Perform site visits daily.
  • Monitor maintenance of sites.
  • Oversee medication maintenance and assist residents in the development of self-medication skills as needed.
  • Provide information regarding community resources and orient residents to the area.
  • Assist in securing vocational assessment services, job training and education programs.
  • Provide advocacy services to gain appropriate entitlements and services.
  • Intervene in situations requiring immediate attention to ensure safety of residents.
  • Provide mediation, counseling, behavioral intervention, and crisis management services.
  • Train and assist residents in the activities of daily living including household maintenance.
  • Provide transportation for residents as needed.
  • Consult and coordinate with clients' family and providers as needed for coordinated service delivery.
  • May occasionally attend team meetings and discharge meetings.
  • Ensure household cleanliness is maintained to agency standards and assist as needed. Conduct fire drills at sites as needed.
  • Notify program supervisor/manager of any medical or psychiatric emergency.
  • Any additional relevant tasks as assigned by management.
