Job Description:

A health services network in New York City is currently seeking a new Care Manager to join their growing team in the Queens. In this role, the Care Manager will be responsible for compliance with all federal, state and accrediting agencies' requirements to ensure the continuum of services necessary for each child in the program. About the Opportunity:

  • State Date: ASAP

  • Schedule: Monday to Friday
  • Hours: 9am to 5pm
  • Call Schedule: None

Responsibilities:The Case Manager (MSW) will:

  • Develop relationships with community organizations and resources to improve service access for children in the program
  • Coordinate treatment services between the CR and community organizations to ensure continuity of care
  • Maintain frequent contact with the children's families and guardians; arrange visits with families and/or siblings; provide services to the child and family and prepare the family for reunification
  • Provide counseling and crisis intervention to all residents
  • Assess client eligibility for SSI, Medicaid, and other third-party funding sources; complete necessary steps to obtain payment, as appropriate
  • Ensure all required documentation for a complete case record, including intake information and consents, admission note, service plans, monthly progress notes, all assessments, service notes, entitlement information and other documentation as required
  • Assist the Intake and Family Resource Specialist and the Program Director with both the intake and pre-placement process and with discharge planning
  • Perform other duties, as needed

Qualifications:

  • 2+ years of professional Child Care and/or direct Clinical Support Services experience
  • Master's of Social Work
  • Solid assessment, clinical, and documentation skills
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized

Desired Skills:

  • Experience as a Clinician for clients with highly specialized needs and multiple psychosocial risk factors
