Are you looking to provide case management to the community? We are hiring a Case Manager (Non-clinical) in the Bronx of NY.

Overview: The Case Manager will provide patient-centered Case Management services to an assigned caseload of patients to help them gain access to care and increase overall health outcomes! This is a part of the Health Home department.

Essential Duties:

• Gathers information to conduct outreach, intake, assessments and reassessments.
• Compete a comprehensive assessment for each patient; conducts HARP and other assessments as needed to address patient health and social needs.
• Provide case management to an assigned caseload of various acuity health home patients.
• Participate in case conferences with the health home team.
• Assess patient eligibility for services and programs.
• Complete relevant documentation to ensure needed services are accessed.
• Document all service information in databases.

Specifics:

Hours: Monday - Friday 8am-4pm or 9a-5pm

Location: Bronx New York 10457

Travel: Will have to visit patients at satellite clinics and the local hospital

Pay: $27.81 - $32.92/hr (based on years of experience)

Job Type: Full Time, Non-exempt

Requirements:

• Bachelors degree
• A minimum of two (2) years experience in case management required.
• Excellent communication skills, verbal and written
• Ability to travel to meet with patients
