Provide targeted skills training in the areas of community living. Provide treatment and specialized case management to clients identified with SMI, who are enrolled in PACT.The Program for Assertive Community Treatment is an evidence-based model of team-based care, comprised of Mental Health Professionals, Case Managers, Nursing, Peers, Psychiatric ARNP's, and Vocational Counseling.Come join our successful team of dedicated professionals, who work diligently and collaboratively together for the purpose of helping one of our community's most vulnerable and misunderstood populations. CLR is an organization that welcomes, engages, and serves individuals with complex wellness needsResponsibilities:Provide both case management and psychiatric rehabilitation services, including, but not limited to assisting consumers with paperwork, self-care, advocacy, and resource acquisition.Use Motivational Interviewing and Cognitive Behavioral therapy in interacting with consumers.Conduct initial and ongoing, comprehensive case manager assessments when needed.Provide support for individuals with psychotic disorders in the community.Provide psychoeducation to clients and their natural supports about Schizophrenia Spectrum Disorders.Provide clinical case coordination, working within a multi-disciplinary team for consumers.Provide group therapy as defined by program needs.Complete paperwork in a timely manner.Keep records up-to-date and comply with WAC, Title XIX and agency requirements.Coordinate services with other program staff, other consultants, relevant agencies, and individuals, e.g. DOC, hospitals, inpatient psychiatric facilities, etc....)Attend treatment team meetings, staff meetings, and designated staff training and consultation sessions.Attend University of Washington fidelity-based training requirements.Share in the rotation of the off-hours crisis phone to support clients (compensation provided)Ensure that ethical and professional standards of practice are maintained.In addition to the above, perform other duties as assigned.Qualifications:Bachelor's degree in social services or other relevant degree; or five years of experience in the behavioral health field.WA State Agency Affiliated credential will be required by hireExperience or willingness to learn how to work with individuals with severe and persistent mental illness, including psychotic disorders.Must be familiar with and willing to work within a harm reduction model.Strong computer skills.Strong analytical, interpersonal, written, and verbal communications skills.Demonstrated ability to multi-task, prioritize deadlines, work independently, take initiative, and maintain confidentiality.Valid Washington State driver's license; proof of insurance.Life Resources is proud to be an Equal Employment Opportunity Employer.
