Tuesday, Wednesday & Thursday 8:AM - 5:PM

The Case Manager coordinates all services necessary to maximize consumer recovery and independence for individuals with mental illness including implementing strategies for obtaining services and supports that are goal-oriented and individualized.

How you will change lives

Case Management

- Ensure consumer involvement in the Person Centered Planning Process (PCP).

- Facilitate the implementation of Individual Plan of Service.

- Perform comprehensive, ongoing psychosocial assessments.

- Participate in legal proceedings including petitioning and testifying in clinical emergencies.

- Provide community based services.

- Facilitate the coordination of consumer intakes, discharges, and transfers.

- Communicate with all service providers in Treatment Plan.

- Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social worker.

- Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.

- Provide referral to appropriate community resources.

We are Hopeful you have

- Bachelor's degree in a human services field from an accredited institution required.

- LLBSW, LBSW, LLMSW, LMSW preferred.

- Prior case management experience in a mental health or rehabilitation setting preferred.

- Prior experience working with Developmentally Disabled population required

- Valid Michigan Driver's License.

Education Required
  • Bachelor of Arts or better in Criminal Justice or related field
  • Bachelor of Social Work or better
  • Bachelor of Human Services or better
  • Bachelor of Science or better in Psychology or related field
  • Bachelor of Science or better in Criminal Justice or related field
Licenses & Certifications Required
