Overview:
The Case Manager performs routine case management work. The Case Manager develops and maintains contact with clients, client families and other service providers in order to coordinate the medical, clinical, social and related needs of clients during treatment. The Case Manager acts as a member of a multi-disciplinary team of treatment providers. This is a full time 40 hour position.
Job Duties and Responsibilities:
- Interview clients to gather information to assess service needs and to assist in the development of an individualized treatment plan for an assigned client caseload.
- Develop and implement discharge plans, service referrals, case management and follow-up for an assigned client caseload.
- Collects urine for routine and random drug testing.
- Provide education components of program, under direction and supervision of Primary Clinician, based on areas of specialty, patient needs, and community history.
- Coordinate service provider activities.
- Identify problem areas, service gaps and barriers to services for clients.
- Documents the client record in accordance with all regulatory and program requirements.
- Acts as a liaison to client, family and aftercare agencies.
- Assist management on policy and procedure changes by making recommendations.
- Participates in meetings, conferences and continuing education programs as appropriate.

Qualifications
Experience in social work. Bachelor's Degree in related field. Education and experience may be substituted for one another.
