Proficiency in relevant software and tools for case tracking and documentation is crucial for effectively managing client cases.The ability to establish rapport with clients and navigate community resources is paramount for successful discharge planning and ongoing support. Additionally, a valid Driver's License is required, as transporting clients may be necessary to facilitate access to essential services.
