Case Manager RN HCA Florida Fort Walton-Destin Hospital Full-Time - Days (8 Hour Shift) / Rotating Weekends HCA Florida Fort Walton-Destin Hospital is a 267-bed hospital. We are a top-performing quality hospital. We have over 300 of the most respected and qualified physicians in the area. We offer an Advanced Primary Stroke Center and a Cancer Care Center. Our services include open-heart surgery and a wide range of specialties. We give advanced care to critically ill patients. We offer emergency services at our main emergency room (ER). We also have a pediatric ER and two freestanding ERs. Together, our team has an unwavering conviction to improve more lives in more ways.Position Summary The RN Case Manager will facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management. The RN Case Manager will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization. The RN Case Manager will identify potential barriers to patient throughput and quality outcomes and will facilitate appropriate discharge plans.These positions are in-person, onsite at Fort Walton-Destin Hospital. There are not remote or Hybrid options.What Qualifications You Will Need: Associates or diploma in nursing required; BSN preferred Florida RN license required; or appropriate compact BCLS-AHA Healthcare Provider certification required within 30 days of employment start date National Case Management Certification (CCM or ACM) preferred Interqual competence required, certification preferredRequired Experience: Prior hospital case management experience preferred. Example: evening is 3p-11p, so one would need to work past 7pm in order to get evening diff.)
