**Type:** Four-month contract, opportunity for extension
**Schedule:** Monday through Friday, 8am to 5pm
**Pay:** $31.50 to $33.55 an hour
**Responsibilities of the Case Manager RN:**
Manages transition of care patients, triages, scheduling, hospital tracking and follow up
Coordinating patient needs with hospice/vitas
Responsible for managing hospital updates, discharges, and scheduling accordingly.
**Qualifications for the Case Manager RN:**
BSN degree, preferred
Active, unrestricted RN licensure in Florida
Prior case management experience is a plus
Bilingual with English and Spanish is a plus
