He/she preforms all duties associated with screening, admission, exit and referral of applicants to include proper written and software related documentation.This staff member assists residents with applications for benefits, community resources and referrals, and provides access to various housing opportunities to facilitate resolution of homelessness.As Housing Navigators, Case Managers collaborate closely with housing providers to maintain an accurate housing referral list and support residents in becoming stably housed. Essential FunctionsActively screen and update list of those seeking services and provide necessary referrals.Case Manage clients and families served, complete screening, intake, assessment, and individualized case plan.Maintain resident information in applicable software, on rosters/forms and compiles case records accurately and in a timely manner.Provides financial management and budget planning support or referrals to include processing of client savings and room fees.Complete regular room checks.Assist residents with access to, and application for benefits, programs, and resources.Coordinate and maintain good communication with local community partners and service providers.Attend and participate in case conference regarding the progress of residents, new admissions, referrals and discharges.Complete data entry as necessary in information management systems.Maintain an up-to-date inventory of housing, employment, and resource opportunities for residents.Provide referrals to residents for community-based services which aide in their stability.Cross-train with other Case Management staff to provide the necessary support and knowledge required in their absence.Attend meetings, appointments and other engagements that pertain to resident case plan resolution.Perform standard screenings for substance use or harmful behaviors and make necessary reports regarding welfare or safety as per policy.Complete files and other documents for client case files. Review case load files (electronic and hard copies) for completeness, and adherence to standard procedures. Process receipts for expenditures in a timely manner and maintain accurate record of documentation.Provide transportation to residents when necessary.Maintain confidentiality and social service code of ethics.Maintain safe and sacred boundaries with program participants and staff members.Perform all other duties as assigned related to your scope of work to include responsibilities at various locations within San Bernardino area administration.Working ConditionsAbility to speak, sit, walk, stand, bend, climb, kneel, and twist on an intermittent basis; to grasp, reach, handle, finger, push or pull objectsAbility to lift up to 20 lbs., or more with assistance.Ability to operate a motor vehicle in daylight, darkness, or in inclement weather conditions.Minimum QualificationsAssociate degree with major study in social work, human services, or related field, or two-years of experience working with similar populations. Applicable life experience in lieu of formal education will be considered.HMIS software or other information management system experience preferred.Working knowledge of mental health, substance abuse, and dual diagnosis treatment philosophies, and modalities.Experience working with families, particularly those who have experienced various forms of trauma.A minimum of one year of Case Management experience required.Experience working with community providers such as police, non-profit organizations, churches, hospital hospitals preferred.Ability to drive and maintain a valid Class C California Driver's License, pass a TSA Driving Test and maintain a clean MVR required.Must be able to satisfactorily pass a background check and DOJ Live Scan.Current CPR Certificate and First Aide Certificate preferred.Skills, Knowledge & AbilitiesKnowledge of or experience with mainstream benefits and services provided within the County of San Bernardino.Experience working with individuals who are experiencing homelessness, justice involved and living with behavioral health diagnosis.Excellent problem-solving skills.Ability to work collaboratively with others on a team.Uses good time management skills and resources to balance responsibilities.Experience operating general office equipment to include computers.
