We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational.Program Description: The Senate is a permanent supportive housing residence for formerly homeless adults, many with mental illness, substance abuse/dependence and co-morbid medical diagnoses. On-site services include medication monitoring, case management, money management, entitlement assistance, medical and psychiatric care, substance abuse counseling, work readiness training, 24 hour/7 day per week front desk security, and group activities.Purpose of Position: The Senate is seeking a dynamic individual to provide case management services to formerly homeless adults diagnosed with a severe persistent mental illness in order for tenants to maintain permanent housing as well as to utilize client centered approach to assist tenants with working towards their goals. Schedule: Monday through Friday 9:00am to 5:00pm orSunday through Thursday Salary: $41,668-$52,085Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate's number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.Roles, Responsibilities, and Essential Duties1.) Provide case management servicesProvide case management to a group of assigned clients; identify short and long-term treatment goals and help clients achieve them, including linkages to community programs such as medical, psychiatric and drug treatment servicesCoordinate and follow up with the onsite medical and psychiatric treatment providersProvide onsite medication monitoring to tenantsAccompany clients on visits to doctors, entitlement offices or other community servicesMaintain case records with documentation of services provided and outcomes, according to specific program proceduresAssist clients in receiving entitlements and in understanding and complying with building rules and government subsidies to maintain residencyCollaborate with staff in other GRCC programs to coordinate client serviceEncourage participation among resistant clients; conduct floor meetings to introduce and encourage program participation2.) Participate as Senate team memberAttend staff meetings, case conferences, training programs and supervision meetings Participate in planning and leading group activities Collaborate to maintain an active resource directoryTrack and maintain all data outcome measures on tenants physical, mental and behavioral health with a results orientated framework Other duties as required include conducting unit inspections, and reviewing changes in government policies as well as changes in pharmaceutical and insurance companiesQualifications/Educational RequirementsBachelor's Degree in social work or a related field requiredExperience working with individuals diagnosed with severe and persistent mental illness, substance abuse, and/or co-occurring disorders preferredSpanish speaking preferred Skills, Knowledge and AbilitiesUnderstanding of housing first modelUnderstanding of severe and persistent mental illnessUnderstanding of harm reduction and trauma informed careAbility to multitaskStrong crisis management skillsStrong interpersonal skillsStrong written and verbal skillsTeam player Computer Skills: To perform this job successfully, an individual should have/be:Proficient in Microsoft Outlook and ExcelAble to use or learn computer databases such as AWARDSPhysical RequirementsSome walking and travel to escort clients to various appointments throughout the city and occasional visits to hospital emergency rooms.Work EnvironmentMostly indoor office, occasional kitchen and basement during groups.
