Job Description AHRC NYC is looking to hire a Case Manager for our Staten Island Employment Business Services (EBS) program. The Case Manager is expected to develop and maintain positive relationships with funding sources, fellow providers, schools, training sites, employers, participants and their families, and staff. This person is responsible to meet the requirements of all contracts to which he/she is assigned and to adhere to all compliance requirements. The Case Manager position has an annual salary of between $38-$42k and comes with a low-to-no cost, comprehensive health benefit package which is just one way that AHRC NYC supports the wellbeing of our employees and their families.   • Create and maintain excellent relationships with all job seekers assigned, in order to provide top quality customer service and ensure future success in employment. Provide coverage within the program in event of excessive staff absences. Counsel participants in both group and individual settings.
  • Ensure program remains in compliance with New York State regulations, Department of Labor regulations, Federal, and Workforce Investment Act. and parent meetings as appropriate.
  • Participate as member of interdisciplinary team, attending case conferences, staff meetings seekers for success in employment. Create and maintain excellent relationships with EBS staff for purpose of preparing job
  • Provide top quality customer service to all job seekers before and after job placements and to employers who hire them, in coordination with Account Executives and other EBS staff.
  • Establish relationships in community and coordinate outreach events for recruitment of new enrollees into programs.
  • Conduct Job Clubs and other pre-employment groups and meetings related to pre- employment issues and/or job seekers.
  • Conduct initial reviews of potential new enrollees and provide guidance through enrollment process to person supported, family and other service providers involved. Perform Discovery process for individuals, as required by supervisor.
  • Participate in Person Centered practices, including Discovery, Community Engagement activities, and Customized Employment.
  • Perform other duties as assigned.
Qualifications
  • Experience or strong capacity in instructing/mentoring program participants, with prior works exposure in teaching or training individuals.
  • Two years of working with the developmentally disabled population preferred.
  • Ability to communicate effectively, both verbally and in written form.
  • Ability to establish and maintain effective working relationships with participants' families, employers, funding sources, Care Coordinators, and other stable holders.
  • Ability to organize work of self and others, as well as work independently as needed.
  • Bachelor's degree preferred.
  • NYS Driver's License preferred.

Schedule:
  • 8 hour shift
