Position Overview: We're looking for a Full time Case Manager for Guardianship who is ready to step up and advocate for incapacitated adults in a meaningful, hands-on way. In this role, you'll not only provide direct support but also leverage technology and innovative solutions to manage client care effectively. Compensation: $27/hour Schedule: Monday-Friday 9AM-5PM, occasionally evening hours Location: Central Islip, NY (in-person ) Reports to: Program Director Key Responsibilities:
- Initiate Client Services: Within 72 business hours of assignment, contact all relevant parties (e.g., legal, social, and healthcare professionals) to gather necessary information and begin the fact-finding process. Use this information to develop a comprehensive service plan that includes social, administrative, fiscal, and legal services tailored to each guardianship client.
- Provide Case Management Support: Offer ongoing case management services, including home visits, telephone reassurance, advocacy, and guidance to clients dealing with issues such as substance abuse, mental health challenges, housing instability, and financial difficulties.
- Client Transportation & Escort: When necessary, transport and escort clients using your own vehicle to appointments for emergency resources or to complete essential applications, ensuring their access to required services.
- Participate in Case Conferencing: Take part in regular case conferencing sessions for collaborative case review and decision-making with the team to ensure comprehensive client support and effective service delivery.
- Maintain Accurate Case Records: Keep detailed and up-to-date progress notes on all assigned cases, documenting key interactions, progress, and any adjustments to the service plan.
- Liaise with Legal and Social Entities: Communicate regularly with judges, referees, lawyers, and DSS caseworkers regarding program participants. Additionally, represent the program in presentations to community referral sources when necessary.
- Complete and Submit Required Reports: Prepare and submit all required reports for clients, including Initial Reports, Interim Reports, Annual Reports, and court updates, ensuring compliance with legal and program standards.
- Billing Coordination: Submit accurate progress notes and relevant documentation to the finance department to support billing processes for services rendered.
- Program Evaluation & Measurement: Develop and implement measurement strategies to assess and evaluate the effectiveness of the guardianship program, identifying areas for improvement and ensuring quality service delivery.
What We are Looking for?
- Bachelor's Degree in social or behavioral sciences (preferred but not required if you have relevant experience).
- At least two years of relevant work experience (social services, case management, or related field).
- Strong proficiency in Microsoft Office, databases, and innovative tech solutions for managing cases.
- Excellent communication skills (both verbal and written) and the ability to connect with diverse communities.
- Valid NY State Driver's License and access to a vehicle for transporting clients when needed.
