Case Manager- Team Leader works with the Case Mangers to ensure that the goals of the Care Management Model are met. Case Manager Team Leader has a working knowledge of the roles and responsibilities of the case manager and their functions in all work areas; execute those roles and responsibilities as needed. Case Manager Team Leader is self-directed, able to take initiative and display ability to think independently and creative. Works closely with Director and has a good working knowledge of the overall operation of the Case Management department. Must have a working knowledge of the perspective payment system, governmental rules and regulation, accreditation agency guidelines. Case Manager Team Leader directs a team of case managers that are responsible for planning, organizing, directing, evaluating and coordinating case management services across the continuum of care. Trains all new employees. Observes training needs and report to Director performance management issues/problems and recommend related actions. Assists with retraining of staff as indicated. Serves as a liaison and resource for other members of the health care team. Coaches, facilitate, solve work problems, and actively participate in daily activities of the team. Collaborates with internal and external customers and assists the Case Management department with managerial activities as needed. Works closely with physician advisors to ensure that utilization review, utilization management, care coordination, and discharge planning issues and or needs are addressed timely. Assembles, analyzes, and types materials related to denials and appeals, etc. and as designated by the Director.Assists the Director with assigned projects. Attends team/leadership meetings and participates in departmental education and tasks as needed.Educattion: Bachelor of Science Degree in Nursing from an accredited non-online university required. Work Experience: Five or more years of experience in clinical nursing required. Four years' experience in Hospital Case Management required.Cerification/Licensure: Current license as a Registered Nurse in the State of Mississippi is required. CCM certification preferred.
