Position Summary: Do you have a passion for child welfare? Are you looking to make a difference in the lives of children in the community? If so, the Residential Case Planner position is the role for you! In this position you will coordinate with local county and state agencies to provide clinical and case coordination to children and families in order to support their long-term goals.

Program Summary: Lee Randall Jones Community Residence, certified by the Office of Mental Health, is an eight-bed residence located in Cheektowaga. The Community Residence provides services to children ages 5-14 with a primary psychiatric disability that does not require acute care but may still need the structure and support of this community-based program. The program provides 24-hour supervision paired with community-based medical, recreational, and psychiatric services that may continue once the child is discharged home.

What will a day as the Residential Case Planner look like?

+ Clinical and case coordination responsibility for children and families on assigned caseload

+ Coordination with local county and state agencies

+ Integration of information from a variety of sources to complete assessments, identify service needs, and complete treatment plan

+ Utilize a variety of treatment interventions including Trauma Focused Cognitive Behavioral Therapy and Family Therapy

+ Present at Service Plan Reviews and ensure follow through of recommendations

+ Maintain case records and other required paperwork in accordance with NYS regulations

+ Demonstrate the ability to engage families in the treatment process

+ Begin discharge planning upon admission and implement diligent efforts toward that end

+ Provide clinical leadership for treatment team by promoting the integration of trauma informed care, collaborative problem solving, and therapeutic crisis intervention principles

+ Participates and contributes to program Quality Improvement process

+ Maintain positive working relationships with program staff and other internal and external customers

+ Complies with all agency policies and procedures

The qualified candidate will have:

+ A Bachelor's Degree in Social Work or related field and experience in the child welfare field, Master's Degree preferred.

+ A valid NYS Drivers License

+ Experience providing trauma treatment to emotionally disturbed children (preferred)

+ The ability to deescalate and manage difficult behaviors of children

+ The ability to initiate or participate in physical interventions when necessary

+ Confidence in standing, walking, climbing, stooping, kneeling and occasional lifting and/or moving more than 25 pounds

Competitive Pay Rate of $25- $27 per hour based on experience
