Job Description:
Primary contact for Continuity of Care services. Provides mental health services for individuals transitioning out of crisis. Work involves conducting interviews and clinical assessments; follow ups with consumers discharging from state hospitals and state contract facilities; provision of continuity of care to referred mental health individuals in crisis. Works under moderate supervision of the Supervisor with limited latitude for the use of initiative and independent judgment in carrying out details of the work. EXAMPLES OF WORK PERFORMED: Provides continuity of care services for individuals transitioning out of crisis Follow up services for individuals coming out of state hospitals and contract facilities Develops comprehensive treatment plans and monitors progress Coordinates referrals to community-based providers Provides crisis supports as deemed necessary Prepares progress notes and other required documentation. KNOWLEDGE, SKILLS, AND ABILITIES: Knowledge of mental illness and emotional components of physical illness; psychiatric diagnosis, psychosocial assessments and crisis intervention. Demonstrated skill in interviewing and reporting results of interviews. Considerable ability to work cooperatively and effectively with professional personnel; to exercise tact, initiate, and use good judgment in dealing with clients and stakeholders; to prepare reports and make recommendations for administrative use; to speak and write effectively; to relate to people and to interact with clients. Must have physical ability to sit for extended periods; working at computers. Ability to lift up to 50 lbs. Some stooping, bending and kneeling. Ability to travel 40% of the time locally and 10% of the time regionally. Must have excellent verbal/written communication. Must have excellent time management skills. Education, Experience, and Training Master's Degree Required. One year case management experience preferred. Fully Licensed LPHA Required or will accept LMSW or LPC-Associate (working on clinicals). Requires successful experience in providing mental health services for persons with severe and persistent mental illnesses, a working knowledge of psychiatric diagnosis, psychosocial assessments and crisis intervention.
