Summary of Position: Responsible for providing Diabetes Self Management Training and support services to patients in individual and group settings. #Referral base is local physicians# practices. #It is essential that a strong commitment to excellent patient care, team work and the use of evidence-based practice exist. Primary Job Responsibilities: Assessment and Implementation: Responsible for conducting thorough diabetes pre-assessments and collaboratively identifying appropriate education needs, goals and interventions with patients. #Monitors and evaluates the effectiveness of these interventions and goal achievement, revising as necessary. #Attention is paid to patient-centered and evidence-based care at all times. #The AADE-7 System will be used as a template for evidence-based patient education. Evaluation: The effectiveness of interventions and specific patient-focused outcomes are measured and evaluated. #They are revised as appropriate to meet the unique needs of each patient. Communication: Document individualized plans of care. #Communicate with referring providers to coordinate patient care. #Effectively and appropriately communicate with patients, family members, staff, professional colleagues and community members in a variety of settings. Education: Provide diabetes self management training and support to patients within the framework of an accredited Diabetes Self Management and Support Program. #Provide patient education in both individual and group settings on a wide range of diabetes self management topics. #Act as a diabetes resource for colleagues, patients, other departments and the Greater Saratoga community. Resources: Seek out educational opportunities to stay current and competent in all areas of work responsibilities and to meet continuing professional education requirements. #Utilize appropriate resources to streamline work processes and solve problems as they arise. #Act as a diabetes resource for colleagues, staff and other professional disciplines. #Responsibly allocate limited resources in an effective and efficient manner. Community: Develop and facilitate community health education programs and exhibits on a variety of diabetes-related topics. #Promote services with potential and current customer groups. Minimum Qualifications: Bachelor of Science in Dietetics or Food # Nutrition. #At least three years# experience in outpatient diabetes setting working with individuals and groups required. #Prior experience with an accredited Diabetes Self Management and Support Program strongly preferred. # Current #NYS licensed registered dietitian. #Certification as Certified Diabetes Educator (CDE) through The National Certification Board for Diabetes Educators required. #Membership in The American Association of Diabetes Educators strongly preferred.# Salary Range: $30.60 - $51.92 Pay Grade: PNB Compensation may vary based upon, but not limited to: overall experience and qualifications, shift, and location.
