BASIC FUNCTIION

To provide routine daily care and services that support the care delivered to patients requiring long term or rehabilitative care, in accordance with the established nursing care procedures and as directed by your supervisor.

CHARACTERISTIC RESPONSIBILITIES AND DUTIES

ESSENTIAL FUNCTIONS

1. Provides care as directed by professional nurse to patients requiring long-term rehabilitative or restorative care.

2. Reports observations and other pertinent information related to the care of the patient.

3. Document objectives information related to patient care.

4. Provides service that support the care delivered to the patient.

5. Provides input in the formulation and evaluation of standards of care.

6. Participates in the development of unit objectives.

7. Participates in the identification of staff educational needs.

8. Conducts self in a professional manner in compliance with unit and facility policies.

9. Participates in the quality assessment and improvement process and activities.

10. Performs other related duties as assigned or requested

11. Is responsible for participation in the training program on the facility immunization program and in the annual vaccination campaign.

EXPOSURE RISK

The Certified Nursing Assistant (CNA) is at high risk for exposure to blood and body fluids or other potentially infectious materials.

SUPERVISION RECEIVED

Receives administrative supervision from the Director of Nursing the Assistant Director of Nursing or the nursing supervisor. May receive functional supervision from Registered or Practical Nurse working on the unit.

SUPERVISION EXERCISED

Exercises functional supervision, responsibility and accountability necessary for carrying out assigned duties.

MINIMUM QUALIFICATIONS

1. Graduation from high school.

2. Previous Nursing Assistant experience or student nurse preferred

3. Completion of a certification program for Nursing Assistants preferred.

MINIMUM PERFORMANCE STANDARDS

Performance in the following areas is acceptable when:

GENERAL PATIENT CARE

1. Patients are approached in a kind, gentle and friendly manner. Respect for the patient's dignity and privacy is consistently provided. Confidentiality of all patient information is maintained.

2. All grievances and complaints mad by the patient are reported to the Charge/ Staff Nurse.

3. Assistance is provided with admissions/ transfers/ discharges.

a. Rooms is striped after patient transferred/ discharged.

b. Patients are moved to accommodate unit needs.

4. Specimens are transported to the appropriate areas.

5. Patient rooms are re-stocked.

a. Rooms are neat and orderly

b. Excess supplies and linen are removed.

6. Care plans are reviewed daily to determine if changes in the patient's daily care routine has been made.

7. Assistance with patient care is provided as directed.

a. Baths are given

b. Linens are changed

c. Patients are properly positioned.

d. AM and PM care is given

e. Vital signs are taken

8. Assistance with restorative programs as indicated.

a. Positioning and alignment

b. Skin integrity

c. Bowel and bladder

d. Ambulation

e. Range of motion

f. Activities of daily

g. Dining

h. Emotional

9. Hailways are cleared of equipment or on one side

10. The unit is maintained in an orderly fashion.

11. Meal trays are distributed and picked up.

12. Patients are assisted or fed at meal times as directed.

13. Bed pans, urinals and emesis basins are distributed and removed as requested

14. Routine specimens are collected as directed.

15. Assistance with treatments is provided as directed.

16. Patient call lights are promptly answered. Appropriate responses to requests are provided.

MAINTENANCE OF SUPPLIES/ EQUIPMENT

1. All equipment is operated in a safe manner and the only equipment utilized that which previous training of use has occurred.

2. Defective equipment is reported to the Charge/Staff Nurse.

3. Vacant rooms are neat in appearance.

4. The nurse's station is maintained

a. Supplies are maintained at par levels and are arranged in an organized manner.

b. Nurse's station is neat and orderly.

c. Full waste containers are emptied.

5. Equipment is accounted for on rounds.

a. Ice containers.

b. Geri-chairs.

c. Over-bed tables.

d. IV poles (if applicable).

e. Stretcher lift (is applicable).

6. Cleaning and maintenance occurs according to schedule.

a. IV or tube feeding pump poles, tube feeding infusion pumps and suction machines are wiped down weekly.

b. Utility room is checked routinely for supplies that need to be returned to the appropriate department, meal trays requiring to dietary, and linen that requires removal to the laundry.

c. Safety hazards are removed from unit floors (liquid spills, food, paper, equipment cords, etc.).

d. Nourishment refrigerator is cleaned and defrosted once a month.

7. The patient environment is safe and clean.

8. Individual patient supplies are re-stocked as indicated.

ADHERENCE TO FACILITY PROCEDURES

1. Procedures are performed according to established method in the procedure manual.

2. Body substance precautions and other appropriate infection control practices are utilized with all activities.

3. Safety guidelines established by the facility (i.e. proper restraint and side rails use) are followed.

4. All accidents or incidents observed are reported on the shift that they occur.

DOCUMENTATION

1. All entries on charts, notes, flow sheets, etc., are recorded in an informative and descriptive manner.

2. Vital signs are recorded accurately on the appropriate form.

3. Intake and output is accurately recorded on the appropriate form.

4. Patient weights are recorded on the appropriate form.

5. Nursing care flow sheet (if applicable) is maintained.

6. Documentation of restorative piogram and progress towards goals. 7. Only LH approved abbreviates are used when recording information.

COMMUNICATION

1. All changes in a patient's condition are reported as soon as possible to the Charge/ Staff Nurse.

2. Staff meetings are attended, if on duty. Minutes are read initialed if not on duty.

3. Patient/unit problems are reported to the Charge Nurse.

PROFESSIONALISM

1. Decisions are made that reflect both knowledge and good judgment.

2. Awareness of own limitation is evident and assistance is sought when necessary.

3. Dress code is adhered to.

4. Committee meetings (if assigned) are attended. Reports related to the committee are given during staff meetings.

5. Responsibility is taken for own professional growth. Attend and participate in all scheduled training, educational and orientation classes, programs and activities including annual mandatory classes.

6. Follow work schedules and complete assigned tasks according to the established policies and procedures of the facility.

7. Notify the facility if you are going to be late or absent from work.

8. Organizational ability and time management is demonstrated.

9. Confidentiality of patient is respected at all times (i.e. when answering telephone and /or speaking to co-workers).

10. Cooperate with inter-departmental personnel to ensure that the needs of the patients are met and maintained.

HUMAN RELATIONS

1. A positive working relationship with patients, visitors and facility staff is demonstrated.

2. Authority is acknowledged and response to the direction of supervisors is appropriate.

3. Time is spent with patients rather than other personnel.

4. Co-workers are readily assisted as needed.

WORKING CONDITIONS

1. Works in the nursing service areas, patient rooms, nurse's station, etc.

2. Sits, bends, stands, lifts and moves intermittently throughout the work hours.

3. Subject to exposure to infectious waste, diseases, conditions and blood and body fluids

4. Subject to frequent interruptions.

5. Attends and participates in continuing educational programs.

6. Communicates with other nursing and departmental personnel.

7. Is involved with patients, peTsonnel, visitors, etc., throughout the working hours.

SPECIFIC REQUIREMENTS

1. Must possess a current, unencumbered license to practice as a CNA in this state.

2. Must beable to read, write and understand the English language at an 8th grade level.

3. Must possess the ability to make independent decisions when necessary. 4'. Must be able to relate information concerning a patient condition.

PHYSICAL AND SENSORY REQUIREMENT

(With or Without the Aid of Mechanical Devices)

1. Must be able to move intermittently throughout the workday.

2. Must be able to speak the English language in an understandable manner.

3. Must be able to cope with the mental and emotional stress of the position

4. Must be able to see and hear or use prosthetics that will enable the senses to function adequately to ensure that the requirements of the position can be fully met (i.e., accurately read measurements on patient related equipment such as thermometers, monitors, gauges).

5. Must be able to function independently, have personal integrity, flexibility, and the ability to work effectively with patients, personnel and support agencies.

6. Must be in good general health and demonstrate emotional stability.

7. Must be able to relate and work with the disabled, ill, elderly, emotionally upset and at times hostile people within the facility.

8. Must be able to lift, push pull, and move a minimum of 50 pounds.

9. Must be able to assist with the evacuation of patients.
