POSITION SUMMARY Provides personal hygiene care to hospice patients following the patient's Plan of Care. ESSENTIAL FUNCTIONS
- Provides personal hygiene care to patients according to the individual Plan of Care, such as bathing and shampooing.
- Deliver routine medical supplies as needed to patients.
- Observe and report any changes in the patient's condition to the Nurse/Care Manager.
- Demonstrate respect and understanding of the philosophy, mission, and service standards of CHOT and for providing excellent customer service.
- Successfully complete 12 hours of in-service training annually.
- May assist patient with transfers as needed.
- Provide compassionate companionship to patients while delivering services.
- Depending on the census, assignments may include patients in the field or inpatient units.
POSITION QUALIFICATIONS
- HS Degree or GED
- Proof of State Certification - Certified Nurse Aide
- 1 year of direct patient care experience required
- Demonstrate skills and written competency prior to working independently with the patient
- Able to operate the cellular device
