This role ensures compassionate, respectful care in alignment with established care plans and patient needs.RESPONSIBILITIES
- Adhere to agency policies and procedures to ensure the highest standards of patient care.
- Provide assigned personal care tasks, including bathing and hands-on support, with compassion and professionalism.
- Maintain timely and accurate documentation for each visit, in compliance with agency standards.
- Communicate any changes in patient condition to the supervising RN promptly.
- Assist with household tasks essential to patient health and well-being, as assigned.
- Support patient mobility and physical activity as directed, including ambulation and exercise.
- Perform basic therapeutic procedures, such as range-of-motion exercises, under supervision.
- Assist with patient medications as directed for self-administered medications.
- Participate in required in-service training and continuing education to maintain skills.
- Engage in patient care conferences and collaborative discussions as scheduled.
RequirementsQUALIFICATIONS
- Completion of a Medicare-approved training program for Home Health Aide and/or Hospice Aide, with demonstrated competency.
- Compassionate and respectful attitude towards the care of the elderly and those who are ill.
- Proficiency in following directions, as well as reading and writing.
- Emotional maturity and resilience to meet the demands of patient care.
WORKING ENVIRONMENTPrimarily works in patient homes with occasional tasks completed at the agency office.
