This CNA position with our Activities Department supports our Long Term Care residents by working Evening Shift, typically from 3:00 p.m. - 8:30 p.m. and will be scheduled every other weekend and every other holiday. Total hours are 20 per week (0.5 FTE) which meets the part time status benefits eligibility.
Job Description:
The Certified Nursing Assistant (CNA) - Activities is responsible for assisting in performing basic care of residents and/or patients, under the general supervision of an RN or Activities Supervisor. CNA - Activities assists in executing the ongoing activity program designed to meet the interests and the physical, mental and psychosocial needs of each resident or patient. The CNA - Activities may also assist with data collection to facilitate the delivery of delegated nursing care.
Major Responsibilities:
1. Leads or assists with group activities, as directed.
2. Visits with residents on a one-to-one basis to provide a friendly visitor; sensory stimulation; validation; individual instruction on specific leisure pursuits; a means of meeting individual needs as outlined in each resident's activity assessment and care plan, etc.
3. Responsible for maintaining a written record of resident's participation and response and/or progress to specific approaches utilized during one-to-one visits.
4. Recognizes abnormal signs/symptoms in residents, which signify a physical change of condition and alerts nursing for assistance.
5. Assist residents by helping pass meal trays, snacks, water, etc.
6. Assists residents with transportation to/from general activities, to/from the dining room, or their room, as requested.
7. Is constantly on alert for conditions that may interfere with the safety/well-being of residents; immediately report hazardous conditions to supervisor and/or charge nurse.
8. Abides by the established policies/procedures of the facility, licensing agencies, as well as, local/state/federal regulatory bodies.
9. Assists with basic care for patients in the areas of nutrition, elimination, comfort, mobility and safety.
10. Transports and transfers patients within the medical complex using appropriate equipment (ex. LIKO ceiling lifts) and utilizing safety precautions as indicated by the patient's health status.
11. Assists nursing staff in lifting and ambulating patients using appropriate practice guidelines.
12. Maintains positive and encouraging environment for residents and coworkers. Responsible for maintaining equipment utilized during shift. As needed, will locate and deliver appropriate equipment to Central Service and Maintenance departments.
13. Works collaboratively with the health care team members to effectively communicate and document accurate information.
14. Demonstrates dependability, organizational skills and effective use of time by setting priorities to complete delegated work within established time frames in accordance with unit CBO/P and unit guidelines.
15. Demonstrates role accountability by adhering to Gundersen Health System Mission statement, policies and confidentiality standards.
16. Assists in the orientation of new personnel.
17. Participates in quality improvement activities in order to continuously improve patient care and unit operations.
18. Participates in Recreation Therapy department meetings and unit staff meetings.
19. All other duties assigned within scope of responsibilities.

Minnesota Certified Nursing Assistant (CNA)
Current Infant-Adult Basic Life Support for Health Care Providers (with hands on learning) from one of the following programs: American Heart Association or American Red Cross.
